目的观察自拟降炎平衡方对重症颅脑损伤患者血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、降钙素原(PCT)及C反应蛋白(CRP)水平的影响。方法将重症颅脑损伤患者160例随机分为观察组与对照组各80例。入院后2组患者均给予对症处理,发病24h后观察组经鼻饲注入自拟降炎平衡方煎剂,对照组同期给予同等剂量的葡萄糖生理盐水鼻饲注入。分别于发病后24h及治疗后第3、5天晨空腹抽取肘静脉血5ml,检测并比较2组血清TNF-α、IL-6、PCT及CRP水平变化。结果治疗前2组血清TNF-α、IL-6、PCT和CRP水平差异无统计学意义(P>0.05)。治疗后,2组患者血清中TNF-α、IL-6、PCT及CRP水平呈先升后降趋势,且观察组波动幅度小于对照组(P<0.05)。结论炎性反应程度与颅脑损伤程度关系密切。自拟降炎平衡方可抑制体内炎性反应水平,调节机体炎性应激反应,有助于重症颅脑损伤患者病情恢复。
Objective To observe the effect of self-made Jiang Yan Fang Heng Fang on serum levels of tumor necrosis factor-α (TNF-α), interleukin-6, procalcitonin (PCT) and C-reactive protein Impact. Methods 160 cases of severe craniocerebral injury were randomly divided into observation group and control group of 80 cases. After admission, the patients in both groups were given symptomatic treatment. After 24 hours after onset, the observation group was injected with self-destructive and balanced prescription decoction via nasal feeding, and the control group received the same dose of normal saline nasal injection. Blood samples of elbow venous blood were drawn from abdominal cavity at 24h after onset and on days 3 and 5 of treatment respectively. The levels of TNF-α, IL-6, PCT and CRP in serum were detected and compared. Results There was no significant difference in serum TNF-α, IL-6, PCT and CRP levels between the two groups before treatment (P> 0.05). After treatment, the serum levels of TNF-α, IL-6, PCT and CRP in both groups increased first and then decreased, and the fluctuation range of the observation group was smaller than that of the control group (P <0.05). Conclusion The degree of inflammatory reaction is closely related to the degree of brain injury. Premeditated anti-inflammatory balance can inhibit the level of inflammatory response in vivo, regulate the body’s inflammatory stress response, contribute to the recovery of patients with severe craniocerebral injury.
