目的研究传染性单核细胞增多症(以下简称传单)患儿实验室检测中EB病毒DNA和异型淋巴细胞的辅助诊断的临床意义。方法采取实时荧光PCR技术来定量检测EB病毒DNA的含量拷贝数(>1.0×103为阳性)和血液室异型淋巴细胞计数检测比例(>4%为阳性)。结果在48例传单患者中,EB病毒DNA>1.0×103(阳性)40例,EB病毒DNA<1.0×103(阴性)8例。异型淋巴细胞>4%(阳性)22例,异型淋巴细胞<4%(阴性)26例。EB病毒DNA阳性率占83.3%;异型淋巴细胞阳性率占45.8%。说明实验室检测对传单患儿有较高的辅助诊断价值。结论 EB病毒DNA定量拷贝数检测有相当高的诊断价值,为临床提供传单患者的快速诊断;异型淋巴细胞检测具有45.8%阳性辅助诊断价值,虽然辅助诊断价值没有核酸检测高,但是对实验室条件要求不高,更适合不具备条件开展分子生物实验室的基层医院开展辅助诊断传单患者。
Objective To study the clinical significance of auxiliary diagnosis of Epstein - Barr virus (EBV) DNA and atypical lymphocytes in laboratory tests of children with infectious mononucleosis (hereinafter referred to as leaflets). Methods Real-time fluorescence PCR was used to quantitatively detect the copy number (> 1.0 × 103) of EBV DNA and the detection rate of abnormal lymphocyte count (> 4% positive) in blood chamber. Results Among the 48 cases, EBV DNA> 1.0 × 103 (positive) in 40 cases and Epstein-Barr virus DNA <1.0 × 103 (negative) in 8 cases. Allogeneic lymphocytes> 4% (positive) in 22 cases, atypical lymphocytes <4% (negative) in 26 cases. EB virus DNA positive rate accounted for 83.3%; atypical lymphocyte positive rate accounted for 45.8%. This shows that laboratory tests have a higher value of auxiliary diagnosis for children with leaflets. Conclusion Quantitative copy number detection of Epstein-Barr virus DNA has a very high diagnostic value for the rapid diagnosis of patients with leaflets in clinical diagnosis. Atypical lymphocyte test has a positive diagnostic value of 45.8%. Although the diagnostic value of the auxiliary nucleic acid is not high, Less demanding, more suitable for those who do not have the conditions to carry out molecular biology laboratory of primary hospitals to develop auxiliary diagnosis leaflets patients.
