目的:探讨偏头痛患儿病变程度和诊断与内皮素(ET)水平以及经颅彩色多普勒(TCD)检测的临床关系。方法:以数字法随机抽选50例偏头痛患儿(5~15岁)和50名健康儿童(5~15岁)为研究对象,采用放射免疫法动态监测儿童体内ET水平变化,患儿间歇期和发作期均二次测定ET水平;采用TCD检测儿童脑血流变化。结果:经TCD检测,观察组患儿脑血管痉挛者27例(54%),各动脉血流速度明显提升,高于对照组患儿(P<0.05);组间比较,观察组患儿发作期和间歇期ET值分别为(86.9±3.5)ng/L和(67.8±4.7)ng/L,前者高于后者,且均高于对照组(P<0.05);组内比较,轻、中度患儿ET水平差异无统计学意义(P>0.05),均低于重度患儿(P<0.05)。结论:偏头痛患儿脑动脉血流明显增快,并且依病变程度和时期不同,体内ET水平存在差异,可辅助临床诊断。
Objective: To investigate the clinical relationship between the severity and diagnosis of migraine children and the level of endothelin (ET) and transcranial color Doppler (TCD). Methods: Fifty children with migraine (aged 5-15) and 50 healthy children (aged 5-15) were enrolled in this study randomly. The radioimmunoassay was used to monitor the change of ET level in children. The ET level was measured twice in both period and attack period. The changes of cerebral blood flow in children were detected by TCD. Results: In TCD group, 27 cases (54%) of cerebrovascular vasospasm in observation group were significantly higher than those in control group (P <0.05). The incidence of cerebral vasospasm in observation group was significantly higher than that in control group (86.9 ± 3.5) ng / L and (67.8 ± 4.7) ng / L respectively, the former was higher than the latter, and both were higher than the control group (P <0.05) There was no significant difference in ET level between children with moderate and severe (P> 0.05). Conclusion: Cerebral arterial blood flow in children with migraine is significantly faster than that in children with migraine. According to the degree and period of disease, the level of ET in the body is different, which may be helpful for clinical diagnosis.
