目的探讨可溶性受体CD163在难治性幼年特发性关节炎(全身型)的早期水平变化及其临床意义。方法收集我院2010年1月至2012年1月确诊为活动期难治性幼年特发性关节炎全身型的患儿33例,将其作为观察组,缓解期30例患儿为观察2组,另选择同期来我院进行体检的健康儿童40例,作为对照组,采用流式细胞术法测定两组T细胞亚群水平,采用ELISA方法测定血清中CD163水平。观察两组患儿CD163的表达水平及与疾病活动性指标的相关性。结果活动期难治性幼年特发性关节炎全身型患儿CD163与免疫T细胞的表达水平均高于对照组,差异有显著性(均P<0.05)。可溶性受体CD163的表达水平与CRP、FER、ESR、WBC、IgE均呈现显著的正相关(P<0.05)。结论 CD163是诊断难治性幼年特发性关节炎全身型较有价值的指标,可为病情活动期进展提供依据。
Objective To investigate the early changes of soluble receptor CD163 in refractory juvenile idiopathic arthritis (systemic) and its clinical significance. Methods Totally 33 children with active idiopathic juvenile idiopathic arthritis who were admitted to our hospital from January 2010 to January 2012 were selected as observation group and 30 children in remission as observation group 2 In the same period, 40 healthy children who were examined in our hospital were selected as the control group. The levels of T lymphocyte subsets in both groups were determined by flow cytometry. The level of CD163 in serum was determined by ELISA. The expression of CD163 in two groups of children and the correlation with disease activity index were observed. Results The expression levels of CD163 and immune T cells in children with active idiopathic juvenile idiopathic arthritis were significantly higher than those in control group (all P <0.05). Soluble receptor CD163 expression levels and CRP, FER, ESR, WBC, IgE showed a significant positive correlation (P <0.05). Conclusion CD163 is a more valuable index for the diagnosis of idiopathic juvenile idiopathic arthritis, which may provide a basis for the progression of disease activity.
