目的通过MRI排粪电影及消化道传输试验检查,评价影像学对便秘的诊断价值,探讨便秘的病因。方法对58例临床诊断便秘的患者先作MRI排粪电影检查,再作消化道传输试验检查,参考王毅、龚水根教授的测量指标及诊断标准作出影像诊断,并与临床及术中结果对比,进行回顾性分析。结果出口梗阻41例,其中直肠前突16例,直肠粘膜脱垂5例,前突合并脱垂7例,盆底痉挛综合征4例,会阴下降综合征3例,多种合并异常6例。消化道传输异常12例,真性慢传输8例,假性慢传输4例。其中,6例为出口梗阻合并传输异常。手术治疗37例,药物治疗21例。结论 MRI排粪电影联合消化道传输试验是一种简单无创的、全面动态的诊断便秘病因的检查方法,能帮助临床医生制定行之有效的治疗方案。
Objective To evaluate the diagnostic value of imaging for constipation and to explore the etiology of constipation through MRI defecography and digestive tract transmission test. Methods 58 cases of clinical diagnosis of constipation patients first MRI defecography film examination, and then digestive tract transmission test examination, with reference to Professor Wang Yi, Professor Gong Shuigen diagnostic criteria and diagnostic criteria for imaging diagnosis, and compared with clinical and intraoperative results, Retrospective analysis. Results Outlet obstruction in 41 cases, including rectocele in 16 cases, 5 cases of rectal mucosal prolapse, prolapse with prolapse in 7 cases, pelvic spastic syndrome in 4 cases, 3 cases of perineal descent syndrome, a variety of combined abnormalities in 6 cases. 12 cases of abnormal gastrointestinal transmission, 8 cases of real slow transmission, 4 cases of false slow transmission. Among them, 6 cases of outlet obstruction combined with transmission abnormalities. Surgical treatment in 37 cases, 21 cases of drug treatment. Conclusion MRI defecography combined with digestive tract transmission test is a simple and noninvasive, comprehensive and dynamic method to diagnose the cause of constipation, which can help clinicians to develop effective treatment options.
