[摘要] 目的 探讨大黄素联合骨髓间充质干细胞对急性肝衰竭大鼠的治疗作用。 方法 选择SD大鼠51只,随机分4组:健康对照组6只,急性肝衰竭(ALF)模型组15只,骨髓间充质干细胞(BMSCs)组15只,大黄素联合骨髓间充质干细胞(EMD BMSCs)组15只。D-氨基半乳糖(D-GalN) 诱导大鼠急性肝衰竭模型; BMSCs组及EMD BMSCs组于造模后12 h尾静脉注射BMSCs悬液1.5 mL(约含BMSCs1.0×106 cells/mL),共1次; EMD BMSCs组于造模后12 h开始腹腔注射大黄素10 mg/kg,每天2次,共3 d。ALF模型组于造模后12 h尾静脉注射等体积无菌PBS。各组大鼠均在造模后72 h,以10%的水合氯醛腹腔注射麻醉,取门静脉血及肝组织标本。测血清ALT、AST、TBil,免疫组化测肝组织Bcl-2、Bax及PCNA蛋白表达。 结果 造模后72 h,ALF模型组大鼠血清ALT、AST和TBil指标显著升高,与健康对照组相比,差异有统计学意义(t=7.191、6.796、6.835,P均<0.05)。与ALF模型组比较,BMSCs组及EMD BMSCs组血清ALT、AST、TBil均有不同程度下降,以EMD BMSCs组为著。EMD BMSCs组中Bcl-2、PCNA表达明显高于健康对照组及ALF模型组,而Bax表达则显著低于ALF模型组,差异有统计学意义(t=5.557,P<0.05)。 结论 大黄素联合骨髓间充质干细胞治疗对急性肝衰竭大鼠肝功能的改善及促进肝细胞再生作用更佳,此种作用可能部分与Bcl-2/Bax的调节有关。
