目的探讨鼻内镜下腺样体切除前、后阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患儿听力及中耳功能的变化。方法选取诊断明确的扁桃体、腺样体肥大所致OSAHS患儿58例,患儿无耳聋家族史,无耳毒性药物应用史。术前纯音测听和声导抗检测患儿中耳功能及听力并记录。全麻下先切除双侧扁桃体,然后在30°内镜下用电动吸割器吸除腺样体,术后给予抗生素及激素全身应用,配合咽鼓管吹张及鼓膜按摩。术后随访3~12个月。结果78%(45/58)OSAHS患儿存在中耳功能异常。术后随访3~12个月,伴听力下降患儿86%听力提高。结论OSAHS伴听力下降患儿行腺样体和扁桃体切除术后听力可获得提高。
Objective To investigate the changes of hearing and middle ear function in children with obstructive sleep apnea hypopnea syndrome (OSAHS) before and after endoscopic adenoidectomy. Methods Fifty-eight children with OSAHS diagnosed by tonsil and adenoid hypertrophy were selected and their family history of deafness and no history of ototoxicity were selected. Preoperative pure tone audiometry and acoustic conductivity detection of middle ear function and hearing in children and record. Under general anesthesia, the bilateral tonsils were resected first, and then the adenoid was aspirated with an electric suction pad under 30 ° endoscopy. After the operation, antibiotics and hormones were given to the body, and combined with eustachian tube blowing and tympanic membrane massage. The patients were followed up for 3 to 12 months. Results 78% (45/58) of the children with OSAHS had middle ear dysfunction. The patients were followed up for 3 to 12 months, with 86% hearing improvement in children with hearing loss. Conclusion The hearing of children with OSAHS with hearing loss can be improved after adenoid and tonsillectomy.
