目的观察重组人碱性成纤维细胞生长因子(rhbFGF)和重组人胰岛素样生长因子-Ⅰ(rhIGF-Ⅰ)与Bio-Oss骨粉复合修复Ⅱ度根分叉牙周组织缺损的效果。方法 2009年4月至2010年1月于中国医科大学盛京医院实验动物中心选取犬4只,人工制备犬上下颌第2、3、4前磨牙的慢性Ⅱ度根分叉病损,共48颗患牙,分为5组,包括3个实验组(A~C组,12颗/组)和2个对照组(D~E组,6颗/组)。A组:Bio-Oss+rhbFGF+rhIGF-Ⅰ;B组:Bio-Oss+rhbFGF;C组:Bio-Oss+rhIGF-Ⅰ;D组:单纯引导组织再生(GTR)术;E组:翻瓣术。分别于植入术前及植入后12周记录颊侧根分叉部位的牙周探针深度(PD)和附着水平(AL)并拍摄X线片,12周后观察牙周组织再生情况。结果多数牙根分叉处PD和AL有不同程度的改善。X线片示A、B、C组牙槽骨几乎充满前磨牙的根分叉,甚至可见较清晰的骨硬板。各实验组均较对照组有更多新生牙槽骨。组织学测量结果可见各实验组新生的牙槽骨高度、牙骨质及结缔组织附着指标均高于对照组;但各实验组间比较,差异均无统计学意义(P>0.05)。结论两种生长因子联合应用与单独应用对促进牙周组织再生无明显差异,但均较单纯GTR术或翻瓣术效果好。
Objective To observe the effect of recombinant human basic fibroblast growth factor (rhbFGF) and recombinant human insulin-like growth factor-I (rhIGF-Ⅰ) and Bio-Oss bone powder in repairing periodontal defects of degree Ⅱ root bifurcation. Methods From April 2009 to January 2010, 4 dogs were selected from Laboratory Animal Center of Shengjing Hospital of China Medical University from April 2009 to January 2010 to prepare chronic Ⅱ degree root bifurcation lesions Teeth teeth were divided into 5 groups, including 3 experimental groups (A ~ C group, 12 / group) and two control group (D ~ E group, 6 / group). Group A: Bio-Oss + rhbFGF + rhIGF-Ⅰ; Group B: Bio-Oss + rhbFGF; Group C: Bio-Oss + rhIGF-Ⅰ; Group D: Simple Guided Tissue Regeneration Surgery. Periodontal probe depth (PD) and attachment level (AL) of the buccal root bifurcation were recorded before implantation and at 12 weeks after implantation, and X-ray films were taken. Periodontal tissue regeneration was observed after 12 weeks. Results Most of the root bifurcation of PD and AL improved to varying degrees. X-ray showed A, B, C alveolar bone is almost full of premolar root bifurcation, and even visible cleft bone hard board. Each experimental group had more new alveolar bone than the control group. Histological measurements showed that alveolar bone height, cementum and connective tissue adhesion index were significantly higher in experimental group than those in control group. However, there was no significant difference between experimental groups (P> 0.05). Conclusion The combined application of two kinds of growth factors and single application have no significant difference on the promotion of periodontal tissue regeneration, but both of them are more effective than simple GTR or flap surgery.
