目的探讨高尿酸血症(HUA)的危险因素及其与代谢性疾病的相互关系。方法采用自行设计的调查表,收集研究对象的基本信息及饮食情况;同时收集实验室检测结果,包括空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(LDL-C)和低密度脂蛋白胆固醇(LDL-C)等指标;探讨诱发HUA的危险因素,同时分析两组人群FPG、血清TG、TC、HDL-C和LDL-C水平有无差异,及其与血清尿酸(UA)水平的相关性。结果 (1)单因素分析:经常食用蛋类、豆类、内脏、熟食、海鲜、火锅、烧烤、干果、FPG、TC、TG、LDL-C均是HUA发生的危险因素(OR=1.107、1.511、1.697、1.415、1.772、1.419、1.614、1.384、2.283、1.906、1.186、1.321);经常食用粗粮、奶类和HDL-C是HUA的保护性因素(OR=0.576、0.634、0.663)。(2)多因素分析:经常食用蛋类、豆类、内脏、海鲜、FPG、TC、TG是HUA发生与否的危险因素(OR=2.082、1.606、2.279、2.199、4.201、2.686、3.291);经常食用粗粮是HUA的保护性因素(OR=0.599)。结论适当改善饮食结构,合理控制血糖、血脂,可有效控制血UA水平,减少HUA和痛风的发生。
Objective To investigate the risk factors of hyperuricemia (HUA) and its relationship with metabolic diseases. Methods The self-designed questionnaire was used to collect the basic information and diet of the study subjects. Meanwhile, laboratory test results including fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (LDL-C), low density lipoprotein cholesterol (LDL-C) and other indicators; to explore the risk factors for the induction of HUA, at the same time analysis of the two groups of people FPG, serum TG, TC, HDL-C and LDL- And its correlation with serum uric acid (UA) levels. Results (1) Univariate analysis showed that regular consumption of eggs, beans, viscera, cooked food, seafood, hot pot, barbecue, dried fruits, FPG, TC, TG and LDL-C were all risk factors of HUA (OR = 1.107,1.511 , 1.697,1.415,1.772,1.419,1.614,1.384,2.283,1.906,1.186,1.321); regular consumption of coarse grains, milk and HDL-C are the protective factors of HUA (OR = 0.576,0.634,0.663). (2) Multivariate analysis: regular consumption of eggs, beans, viscera, seafood, FPG, TC and TG were the risk factors for the occurrence of HUA (OR = 2.082,1.606,2.279,2.199,4.201,2.686,3.291); Regular consumption of coarse grains is a protective factor for HUA (OR = 0.599). Conclusion Appropriate improvement of dietary structure, rational control of blood glucose and blood lipids can effectively control blood UA levels and reduce the occurrence of HUA and gout.
