目的:分析驻高原山地武警官兵口腔疾病谱,为有针对性地制定口腔医疗保障方案提供参考。方法:选择在某武警医院口腔科诊治的驻高原山地武警官兵502例次,收集所有入选病例门诊就诊,包括年龄、军龄、部职别、就诊时间、诊断及治疗方法等资料,参照相关标准对口腔疾病进行分类整理,并录入Excel 2010软件系统进行统计分析。结果:(1)本组502例次中,口腔疾病居前3位的依次为口腔颌面部感染218例次(43.4%)、牙周疾病110例次(22.0%)和口腔黏膜病71例次(14.2%),其就诊例次占总例次的79.5%;位列其后的口腔疾病包括龋病54例次(10.8%)、口腔颌面部创伤36例次(7.1%)及其他类型13例次(2.5%)。其中,口腔颌面部感染以急性下颌智齿冠周炎为主,占49.0%(107/218);牙周疾病以慢性牙龈炎为主,占58.0%(64/110);口腔黏膜病以唇炎为主,占60.6%(43/71);龋病以深龋为主,占63.0%(34/54);口腔颌面部创伤以牙折断为主,占44.4%(16/36)。急性下颌智齿冠周炎10例引发嚼肌间隙感染和脓肿,经抗感染、脓肿切开引流术等对症治疗后痊愈;慢性牙龈炎8例发展为慢性牙周炎,导致多牙Ⅱ°或Ⅲ°松动,经定期牙周超声洁治、呋喃西林液含漱及碘甘油牙周涂布等治疗后,病情得到有效控制;深龋7例引发急(慢)性牙髓炎和根尖周炎,经局麻下开髓引流、安抚镇痛,行根管治疗术后痊愈。(2)警官就诊85例次,以牙周疾病为主;士官就诊241例次,以口腔颌面部感染为主;士兵就诊176例次,以口腔黏膜病和口腔颌面部感染为主。结论:驻高原山地武警官兵口腔疾病谱具有其自身特点,需根据口腔疾病类型、高发病种及人群分布特点等有针对性地采取口腔医疗保健措施。
OBJECTIVE: To analyze the oral disease spectrum of officers and soldiers in mountain armed police station in the plateau, and to provide a reference for the formulation of oral medical insurance plan. Methods: A total of 502 cases of officers and soldiers of the mountain armed police stationed in the Stomatology Department of a Armed Police Hospital were selected to collect outpatient visits of all the selected cases, including age, military age, departmental level, treatment time, diagnosis and treatment methods. Results: (1) In the group of 502 cases, the top three oral diseases were oral and maxillofacial infection in 218 cases (43.4%), periodontal disease in 110 cases (22.0%) and oral mucosal disease in 71 cases (14.2%) received 79.5% of the total cases, followed by oral diseases including 54 cases of dental caries (10.8%), 36 cases of oral and maxillofacial trauma (7.1%) and other Type 13 cases (2.5%). Among them, oral and maxillofacial infection was mainly acute pericoronitis (49.0%, 107/218). Periodontal disease was mainly chronic gingivitis, accounting for 58.0% (64/110). Oral and maxillofacial injuries were mainly tooth fractures, accounting for 44.4% (16/36). Acute mandibular wisdom teeth pericoronitis in 10 cases caused the masseter muscle interstitial infection and abscess, anti-infection, abscess incision and drainage after symptomatic treatment recovered; chronic gingivitis in 8 cases of chronic periodontitis, resulting in multiple teeth Ⅱ ° or Ⅲ ° loosening, the periodontal ultrasound cleaning, nitrofurazone rinse and periodontal iodine glycerol coating and other treatment, the disease was effectively controlled; deep caries in 7 cases caused by acute (slow) pulpitis and apical periodontitis, Under local anesthesia open pulp drainage, appease pain relief, root canal treatment cured. (2) The police officers visited 85 cases for the periodontal disease, 241 were visited by noncommissioned officers, mainly oral and maxillofacial infections. The soldiers visited 176 cases, mainly oral mucosal diseases and oral and maxillofacial infections. Conclusion: The oral disease spectrum of officers and soldiers in the mountain armed police stationed in the plateau has its own characteristics. The oral health care measures should be taken according to the types of oral diseases, the characteristics of high incidence and the distribution characteristics of the population.
