目的分析CD1a和CD1b分子在结核性和恶性胸腔积液中阳性表达,探讨其在胸腔积液中诊断及鉴别诊断价值。方法选取临床明确诊断的病例67例,其中结核性胸膜炎31例,恶性肿瘤病例36例。分别留取胸腔积液10ml及其外周血2ml,应用流式细胞仪检测CD1a和CD1b的阳性表达。结果结核性胸腔积液组和恶性胸腔积液组外周血中CD1a和CD1b的阳性表达无统计学差异(P>0.05);结核性胸膜炎和恶性肿瘤患者胸腔积液中CD1a和CD1b的表达明显高于其外周血(P<0.01);恶性胸腔积液中CD1a和CD1b的阳性表达明显高于结核性胸腔积液(P<0.05)。若以CD1a和CD1b的80%阳性表达为临界值,对恶性胸腔积液诊断的灵敏度和特异度分别为87%和62%,约登指数为49%。结论 CD1a和CD1b的阳性表达可以作为结核性和恶性胸腔积液鉴别诊断的参考依据。
Objective To analyze the positive expression of CD1a and CD1b in tuberculous and malignant pleural effusion, and discuss the diagnostic and differential diagnostic value of CD1a and CD1b in pleural effusion. Methods Sixty-seven cases with clinically definite diagnosis were selected, including 31 cases of tuberculous pleurisy and 36 cases of malignant tumor. The pleural effusion were collected 10ml and its peripheral blood 2ml, the use of flow cytometry CD1a and CD1b positive expression. Results There was no significant difference in the expression of CD1a and CD1b between tuberculous pleural effusion group and malignant pleural effusion group (P> 0.05). The expression of CD1a and CD1b in tuberculous pleural effusion and malignant tumor was significantly higher (P <0.01). The positive expressions of CD1a and CD1b in malignant pleural effusion were significantly higher than those in tuberculous pleural effusion (P <0.05). Sensitivity and specificity for the diagnosis of malignant pleural effusion were 87% and 62%, respectively, and the Youden index was 49%, assuming a positive expression of 80% of CD1a and CD1b as a cut-off value. Conclusion The positive expression of CD1a and CD1b can be used as a reference for the differential diagnosis of tuberculous and malignant pleural effusion.
