目的 探讨Ⅳ型胶原酶和Ⅳ型胶原在甲状腺乳头状癌中 (PTC)的表达及意义。方法 免疫组化SP法检测Ⅳ型胶原酶和Ⅳ型胶原的表达。结果 ⑴Ⅳ型胶原酶在PTC、微小癌、腺内型、腺外型和癌旁组织中的阳性率分别为 77 9%、66 7%、80 0 %、88 9%和 2 5 4%;Ⅳ型胶原酶在PTC与癌旁组织间 ;淋巴结转移 ( 97 1%)与无转移PTC组 ( 66 7%)间的阳性表达存在显著性差异(P <0 0 1)。⑵Ⅳ型胶原在PTC( 4 3 2 %)与癌旁 ( 79 7%) ;微小癌 ( 63 0 %)、腺内型 ( 3 8 0 %)与腺外型 ( 2 7 8%)阳性表达比较具有显著性差异 (P <0 0 5 )。⑶Ⅳ型胶原酶和Ⅳ型胶原在PTC共同阳性表达 2 1例 ,共同阴性 1例 ,两者表达呈负相关 (γ =-0 5 60 ,P <0 0 1)。结论 Ⅳ型胶原酶具有溶解Ⅳ型胶原、促进肿瘤转移的功能 ,Ⅳ型胶原酶高表达和Ⅳ型胶原表达下降在PTC的侵袭、转移中发挥重要作用
Objective To investigate the expression and significance of type IV collagenase and type IV collagen in papillary thyroid carcinoma (PTC). Methods Immunohistochemical SP method was used to detect the expression of type IV collagenase and type IV collagen. Results (1) The positive rate of type IV collagenase in PTC, microcarcinoma, glandular, glandular, and paracancerous tissues was 77 9%, 66 7%, 80 0%, 88 9%, and 254%, respectively; IV There was a significant difference in the positive expression of type II collagenase between PTC and paracancerous tissues; lymph node metastasis (97 1%) and non-metastatic PTC group (66 7%) (P < 0 01). (2)Comparison of the positive expression of type IV collagen in PTC (4.32%) and para-carcinoma (79.7%); microcarcinoma (.63%), glandular type (38%), and glandular type (27.8%). There was a significant difference (P < 0 05). (3) Type IV collagenase and type IV collagen were positively expressed in PTC in 21 cases, and were negative in 1 case. The expression of collagenase type IV was negatively correlated (γ = 0 5 60, P <0 01). Conclusion Type IV collagenase has the function of dissolving type IV collagen and promoting tumor metastasis. High expression of type IV collagenase and decreased expression of type IV collagen play an important role in the invasion and metastasis of PTC.
