目的分析宫腔球囊压迫和宫腔内纱布填塞止血的疗效,比较产后出血患者经这两种止血方法处理后的预后情况。方法 58例产后出血患者,根据止血方法不同分为宫腔球囊压迫组(30例)和宫腔内纱布填塞组(28例)。比较两组患者止血操作时间、总治疗时间、治疗24 h后的血红蛋白水平、止血率和子宫动脉结扎率。术后1个月对患者进行随访,观察患者的子宫恢复情况。结果宫腔球囊压迫组的止血操作时间和总治疗时间均显著短于宫腔内纱布填塞组,治疗24 h后的血红蛋白水平显著高于宫腔内纱布填塞组(P<0.05);宫腔球囊压迫组成功止血26例(86.7%),子宫动脉结扎3例(10.0%);宫腔内纱布填塞组成功止血13例(46.4%),子宫动脉结扎10例(35.7%);两组患者止血率和子宫动脉结扎率比较,差异均有统计学意义(P<0.05)。随访1个月,宫腔球囊压迫组并发症发生率低于宫腔内纱布填塞组(P<0.05)。结论与宫腔内纱布填塞治疗相比,宫腔球囊压迫可以达到更好的治疗效果,且将造成感染的可能性降低。
Objective To analyze the curative effect of intrauterine balloon compression and intrauterine gauze packing to compare the prognosis of postpartum hemorrhage patients treated by these two hemostatic methods. Methods Fifty-eight postpartum hemorrhage patients were divided into intrauterine balloon compression group (30 cases) and intrauterine gauze stuffing group (28 cases) according to the method of hemostasis. Hemostatic operation time, total treatment time, hemoglobin level, hemostasis rate and uterine artery ligation rate after 24 h treatment were compared between the two groups. Patients were followed up 1 month after surgery to observe the patient’s uterus recovery. Results The hemostatic operation time and total treatment time of uterine balloon compression group were significantly shorter than that of intrauterine gauze packing group. The hemoglobin level after 24 h treatment was significantly higher than that of intrauterine gauze packing group (P <0.05) In the balloon compression group, hemostasis was achieved in 26 cases (86.7%) and uterine artery ligation in 3 cases (10.0%). Intrauterine gauze packing group was successfully stopped in 13 cases (46.4%) and uterine artery ligation in 10 cases (35.7% Hemostasis rate and uterine artery ligation rate, the difference was statistically significant (P <0.05). One month follow-up, the incidence of complications in uterine balloon compression group was lower than that in intrauterine gauze packing group (P <0.05). Conclusion Compared with intrauterine gauze packing, uterine balloon compression can achieve better therapeutic effect, and will reduce the possibility of infection.
