目的:探讨无创机械通气治疗在慢性阻塞性肺疾病合并II型呼吸衰竭的临床疗效。方法:48例COPD合并II型呼吸衰竭住院患者随机分为通气组与对照组各例均接受常规抗感染、祛痰、平喘等内科药物治疗通气组患者加用无创机械通气治疗。结果:两组患者治疗后的血气较治疗前均有明显改善(P<0.05)而与对照组相比,通气组患者治疗后血气改善更明显(P<0.05);无创机械通气组中耐受良好者24例1例不能耐受。与对照组相比血气改善时间明显缩短(P<0.01),且治疗组患者血压水平未受到明显影响(P>0.05);也未发生气压伤或呼吸机相关肺炎。结论:无创机械通气是治疗COPD合并II型呼吸衰竭的一种比较安全有效的方法。
Objective: To investigate the clinical effect of noninvasive mechanical ventilation on chronic obstructive pulmonary disease with type II respiratory failure. Methods: Forty-eight inpatients with COPD and type II respiratory failure were randomly divided into ventilated group and control group. All patients received conventional anti-infective, expectorant, antiasthmatic and other medical treatment ventilation group patients with non-invasive mechanical ventilation. Results: Compared with the control group, the blood gas levels of the two groups were significantly improved after treatment (P <0.05), and the improvement of blood gas was more obvious in the ventilation group after treatment (P <0.05). The tolerance in the non-invasive mechanical ventilation group 24 cases of good one case can not tolerate. Compared with the control group, the improvement time of blood gas was significantly shorter (P <0.01), and the blood pressure level of the treatment group was not significantly affected (P> 0.05). No barotrauma or ventilator-associated pneumonia occurred. Conclusion: Noninvasive mechanical ventilation is a safe and effective method for the treatment of COPD with type II respiratory failure.
