目的初步探讨PPAR-γ2基因Pro12Ala位点多态性与江西地区汉族人糖尿病视网膜病变(DR)的相关性。方法临床随机筛选出无亲缘关系的研究对象共185例,其中,实验组为根据WHO1999年糖尿病诊断标准和糖尿病视网膜病变诊断标准,诊断为糖尿病视网膜病变的住院和门诊患者66例(男32例,女34例,平均年龄57.94±11.56岁);健康对照组119例(男57例,女62例,平均年龄58.54±12.56岁)。采集研究对象的一般临床特征,应用全自动生化分析仪测定TC、TG、HDL-C、LDL-C、FPG、2hPG等临床指标,放免法测定空腹胰岛素水平,高效液相法测定HbA1c。运用实时荧光PCR方法鉴定研究对象PPAR-γ2基因Pro12Ala基因型;统计学处理运用SPSS11.5统计学软件进行数据统计分析,计数资料间的比较用卡方检验,组间计量资料均以均数±标准差(-x±s)表示,用Logistic回归分析多因素相关性。结果 1.实验组和健康对照组基因型均为PP纯合子,Pro12Ala基因型频率及等位基因频率分布差异均无统计学意义(P>0.05);2.实验组与健康对照组临床资料比较:实验组SBP、FPG、2hPG、HbA1c、FI NS、HOMA-IR均高于健康对照组,HDL-C低于健康对照组,差异有统计学意义(P<0.05);3.Logistic回归分析发现HbA1c为糖尿病视网膜病变发生的独立危险因素。结论 1.PPAR-γ2基因Pro12Ala位点多态性与部分江西地区汉族人2型糖尿病无明显相关性。2.HbA1c为糖尿病视网膜病变发生的独立危险因素。
Objective To investigate the association between PPAR-γ2 gene Pro12Ala polymorphism and diabetic retinopathy (DR) in Han nationality in Jiangxi province. Methods A total of 185 unrelated subjects were screened out randomly in our study. Among them, 66 cases of inpatients and outpatients with diabetic retinopathy were diagnosed according to WHO criteria for diagnosis of diabetic nephropathy and diabetes mellitus, 34 females, with an average age of 57.94 ± 11.56 years); 119 healthy controls (57 males and 62 females, mean age 58.54 ± 12.56 years). The clinical features of the subjects were collected. The clinical indexes such as TC, TG, HDL-C, LDL-C, FPG and 2hPG were measured by automatic biochemical analyzer. The fasting insulin level was determined by radioimmunoassay and HbA1c by HPLC. The real-time fluorescent PCR method was used to identify the genotypes of Pro12Ala in PPAR-γ2 gene. Statistical analysis was performed using SPSS11.5 statistical software for statistical analysis. Chi-square test was used to compare the count data. Standard deviation (-x ± s) that Logistic regression analysis of multi-factor correlation. The genotypes of experimental group and healthy control group were PP homozygote, Pro12Ala genotype frequency and allele frequency distribution difference was not statistically significant (P> 0.05); 2.Experimental group and healthy control group clinical data comparison : The levels of SBP, FPG, 2hPG, HbA1c, FI NS and HOMA-IR in the experimental group were significantly higher than those in the healthy control group, while the levels of HDL-C in the experimental group were lower than those in the healthy control group (P <0.05) .3Logistic regression analysis HbA1c is an independent risk factor for diabetic retinopathy. Conclusion 1.PPAR-γ2 gene Pro12Ala polymorphism and some Jiangxi Han Chinese type 2 diabetes no significant correlation. 2.HbA1c is an independent risk factor for diabetic retinopathy.
