目的:分析椎管内肿瘤的MRI表现,寻找具有诊断和鉴别诊断意义的征象。方法:回顾性分析2000年至2005年在我院行MRI检查并经过手术病理证实的38例椎管内肿瘤病例。MRI检查包括常规T1WI、T2WI序列,部分患者还行矢状脂肪抑制T2WI扫描。增强对比剂为钆喷酸葡甲铵(Gd-DTPA),剂量0.2mmol/kg,静脉注射。结果:8例髓内肿瘤中室管膜瘤4例;星形细胞瘤3例;血管母细胞瘤1例。25例髓外硬膜下肿瘤中神经源性肿瘤14例;脊膜瘤11例。硬膜外肿瘤5例,转移瘤3例,脂肪瘤2例。结论:MRI在椎管内占位性病变的诊断中有准确的定位能力,增强扫描能提高椎管内肿瘤的检出率,并有重要的定性诊断价值。
OBJECTIVE: To analyze MRI findings of intraspinal tumors and to look for signs that have diagnostic and differential diagnostic significance. Methods: A retrospective analysis of 38 cases of spinal canal tumor confirmed by MRI in our hospital from 2000 to 2005 was performed. MRI examination, including conventional T1WI, T2WI sequence, some patients also sagittal fat suppression T2WI scan. Enhanced contrast agent for gadopentetic acid meglumine (Gd-DTPA), a dose of 0.2mmol / kg, intravenously. Results: There were 4 cases of ependymoma in 8 cases of intramedullary tumors, 3 cases of astrocytoma and 1 case of hemangioblastoma. Neurogenic tumors in 25 cases of extramedullary subdural tumors in 14 cases; 11 cases of meningiomas. Epidural tumors in 5 cases, 3 cases of metastases, lipoma in 2 cases. Conclusion: MRI has accurate localization in the diagnosis of space-occupying lesions in spinal canal. Enhanced scanning can improve the detection rate of spinal canal tumors and has important diagnostic value.
