目的 探讨康复治疗对老年头颈部肿瘤放疗患者营养状态的影响.方法 49 例接受根治性放疗的老年头颈部肿瘤患者随机分为康复治疗组(24 例)和对照组(25 例) .对照组给予常规的健康教育, 康复治疗组给予吞咽功能训练、心理疏导和营养指导, 分析比较两组患者在放疗前、放疗结束时、放疗后3 个月的体重、血红蛋白、白蛋白、前白蛋白和营养状态.结果 放疗结束时与放疗后3 个月, 对照组的平均体质量低于放疗前和康复治疗组(P <0. 05).放疗结束时、放疗后3 个月, 对照组的血红蛋白、白蛋白和前白蛋白均低于放疗前(P <0. 05); 放疗结束时, 康复治疗组的白蛋白和前白蛋白均低于放疗前, 但高于对照组(P<0. 05); 放疗后3 个月, 康复治疗组的血红蛋白、白蛋白和前白蛋白均高于对照组( P < 0. 05 ) .放疗前康复治疗组和对照组营养状况评分≥2 分的患者分别为 19 例(79. 2% )和20 例(80. 0% ); 放疗结束时、放疗后3 个月, 对照组的营养状况评分差于放疗前和康复治疗组(P<0. 05); 放疗结束时, 康复治疗组的营养状况评分差于放疗前(P <0. 05).结论 康复治疗可以改善老年头颈部肿瘤放疗患者的营养状态.“,”Objective To explore the effect of rehabilitation therapy on nutritional status in elderly patients with head and neck cancer receiving radiotherapy. Methods 49 elderly patients with head and neck cancer receiving radical radiotherapy were ran-domly divided into control group (CG) and rehabilitation therapy group (RG). The CG (25 cases) received health education only. The RG (24 cases) received swallowing training, psychological counseling and dietary guidance. Body weight, hemoglobin, albu-min, prealbumin, and nutritional status were analyzed before radiotherapy, at the end of radiotherapy and 3 months after radiothera-py. Results At the end of radiotherapy and 3 months after radiotherapy, the body weight in CG was significantly lower than that be-fore radiotherapy and that in RG (P<0. 05). The levels of hemoglobin, albumin and prealbumin in CG were lower than those before radiotherapy (P<0. 05). At the end of radiotherapy, the levels of albumin and prealbumin in RG were lower than those before radio-therapy (P<0. 05), but higher than those in CG (P<0. 05). Three months after radiotherapy, the levels of hemoglobin, albumin and prealbumin in RG were higher than those in CG (P<0. 05). Before radiotherapy, there were 19 cases (79. 2% ) with nutrition-al status scores≥2 in RG and 20 cases (80. 0% ) in CG. At the end of radiotherapy and 3 months after radiotherapy, nutritional sta- tus score in CG was worse than that before radiotherapy and that in RG ( P<0. 05). At the end of radiotherapy, nutritional status score in RG was worse than that before radiotherapy ( P<0. 05). Conclusion Rehabilitation therapy can effectively improve nutri-tional status in elderly patients with head and neck cancer receiving radiotherapy.
