目的探讨组织蛋白酶D(Cath-D)、增殖细胞核抗原(PCNA)、雌激素受体(ER)和孕激素受体(PR)在不同类型子宫内膜增生组织中的表达。方法 60例子宫内膜增生患者分为不典型增生(16例,AH组)、复杂性增生(18例,CH组)和单纯性增生(26例,SH组);另选取20例正常的增殖期子宫内膜作为对照组。用免疫组化法检测增生子宫内膜组织Cath-D、PCNA、ER和PR的阳性表达率。结果四组ER的阳性表达率相仿(P>0.05)。AH组、CH组增生子宫内膜组织中的PR表达均明显低于对照组(P<0.05);AH组和CH组的Cath-D和PCNA的阳性表达率均明显高于对照组(P<0.01);SH组、CH组、AH组细胞核中Cath-D、PCNA的阳性表达率逐级递增,而PR的表达水平逐渐减弱。结论检测子宫内膜增生组织中Cath-D、PCNA、PR的表达水平有助于评估子宫内膜增生的预后,指导临床治疗。
Objective To investigate the expression of cathepsin D, proliferating cell nuclear antigen (PCNA), estrogen receptor (ER) and progesterone receptor (PR) in different types of endometrial hyperplasia. Methods Sixty patients with endometrial hyperplasia were divided into atypical hyperplasia (AH group), complicated hyperplasia (CH group) and simple hyperplasia (26 cases, SH group). Another 20 cases of normal proliferative Endometrium as a control group. Immunohistochemistry was used to detect the positive expression rates of Cath-D, PCNA, ER and PR in proliferative endometrium. Results The positive rates of ER in the four groups were similar (P> 0.05). The expression of PR in AH group and CH group was significantly lower than that in control group (P <0.05). The positive expression rates of Cath-D and PCNA in AH group and CH group were significantly higher than those in control group (P < 0.01). The positive expression rates of Cath-D and PCNA in nucleus of CH group, CH group and AH group increased gradually, while the expression of PR decreased gradually. Conclusion Detecting the expression of Cath-D, PCNA and PR in endometrial hyperplasia tissue is helpful to evaluate the prognosis of endometrial hyperplasia and guide the clinical treatment.
