目的:观察分析2型糖尿病患者血清脂蛋白(Lp(a))的浓度水平与患者24h尿白蛋白排泄率(UAER)水平的相关性,探讨Lp(a)浓度水平的检测对2型糖尿病肾病早期诊疗的意义。方法:选择135例2型糖尿病患者作为研究对象,根据UAER水平随机分为正常白蛋白尿组、微量白蛋白尿组和大量白蛋白尿组,并选取50例健康人士作为正常对照组。检测对比分析Lp(a)与UAER水平。结果:糖尿病组的Lp(a)浓度水平明显高于对照组,差异具有统计学意义(P<0.05);微量白蛋白尿组和大量白蛋白尿组的Lp(a)浓度水平明显高于正常白蛋白尿组,差异具有统计学意义(P<0.05),且随2型糖尿病肾病患者病情的进展,Lp(a)浓度水平会相应升高,患者的Lp(a)水平与UAER水平呈正相关(P<0.01)。结论:2型糖尿病肾病患者Lp(a)浓度水平与其病情的进展呈明显正相关性,Lp(a)水平检测对早期2型糖尿病肾病的诊断和及时治疗有着重要意义。
Objective: To investigate the correlation between serum lipoprotein (Lp (a)) level and urinary albumin excretion rate (24h) in patients with type 2 diabetes mellitus (T2DM) and to explore the relationship between Lp (a) level and type 2 diabetic nephropathy Significance of early diagnosis and treatment. Methods: One hundred and thirty-five patients with type 2 diabetes mellitus were selected as study subjects and randomly divided into normal albuminuria group, microalbuminuria group and massive albuminuria group according to the level of UAER, and 50 healthy individuals were selected as normal control group. The levels of Lp (a) and UAER were detected and compared. Results: The level of Lp (a) in diabetic group was significantly higher than that in control group (P <0.05). The levels of Lp (a) in microalbuminuria group and large albuminuria group were significantly higher than those in normal group Albuminuria group, the difference was statistically significant (P <0.05), and with the progression of patients with type 2 diabetic nephropathy, Lp (a) concentration levels will be increased accordingly, the patient’s Lp (a) level and UAER level was positively correlated (P <0.01). Conclusion: The level of Lp (a) in patients with type 2 diabetic nephropathy has a positive correlation with the progression of the disease. The detection of Lp (a) level is of great importance in the diagnosis and early treatment of type 2 diabetic nephropathy.
