One of the goals of treating ARDS is to improve its gas exchange and tissue oxygen release, while PEEP improves its arterial oxygenation but impairs cardiac output. It is currently thought that increased pulmonary vascular resistance (PVR) is one of the limiting cardiac function factors, and its importance is still debated. Recently, the focus of attention has been on the pulmonary circulation in these patients, but there is still debate about whether there is an increase in PVR due to different etiologies and stages of disease progression. However, the increase in PVR is much greater than the increase in systemic vascular resistance (SVR) and is related to the severity of ARDS and its prognosis.
