目的:研究正畸方法直立阻生磨牙后前移关闭缺失第一恒磨牙间隙的疗效。方法:样本共计32例,下颌第一恒磨牙已拔除或无保留价值,且伴同侧下颌第三磨牙阻生。口内辅弓法直立阻生磨牙,直丝弓矫治技术关闭下颌第一磨牙间隙。治疗前后检查第二恒磨牙和第三恒磨牙附着丧失(Attachment Loss AL),统计使用SPSS 13.0统计学软件包,分析采用非参数检验附着丧失的变化。结果:32例阻生齿均直立,下颌第一恒磨牙缺隙关闭。X线片检查示:牙根平行良好,阻生齿周围骨质再生良好,牙槽嵴形态正常,牙周膜清晰。所有病例均取得满意的疗效。治疗后较治疗前出现少量附着丧失量增加,差异具有显著性(P<0.05)。结论:下颌第一磨牙缺失后可通过正畸治疗移动第二磨牙且直立前倾阻生第三磨牙前移代替原第一、二磨牙,疗效较好,可见少量附着丧失,建议治疗前评估患者附着龈水平和牙周状况,降低治疗风险。
OBJECTIVE: To study the curative effect of orthodontic approach to erect the impacted molars and close the missing first permanent molars. Methods: A total of 32 samples were obtained. The first mandibular permanent molars were removed or without reservation, and the ipsilateral mandibular third molars were impacted. Oral implants orthostatic impacted molars, straight wire arch correction technology to close the first molar gap. Before and after treatment, the second and third permanent molars were examined for Attachment Loss AL (ALS). The statistics were analyzed using the SPSS 13.0 statistical software package and the non-parametric test was used to test for changes in attachment loss. Results: All 32 impacted teeth were erect, and the first permanent mandibular molar gap was closed. X-ray examination showed: root parallel well, good regeneration of bone around the impacted teeth, alveolar ridge morphology, periodontal ligament clearly. All cases have achieved satisfactory results. After treatment, there was a small increase in the amount of attachment loss before treatment, the difference was significant (P <0.05). CONCLUSION: After the first mandibular molar loss, the second molars can be moved by orthodontic treatment and the third molars impacted forward can be used to replace the first and second molars. The effect is good and a small amount of attachment loss can be seen. It is recommended to evaluate patients before treatment Gingival attachment and periodontal conditions, reducing the risk of treatment.
