目的通过临床对照试验研究论证附着龈重建在口腔种植修复中的应用价值,为临床提供参考。方法选取口腔科2010年3月—2014年5月入院接受附着龈重建术治疗患者120例作为研究对象,按照不同的治疗方法将120例患者均分为观察组和对照组,每组患者60例。观察组患者进行附着龈重建术治疗,即在牙齿正中偏向舌的一侧对患者的牙槽嵴部位进行切口;对照组患者采用传统的治疗方式,在正中部位切口进行手术治疗,附着龈重建术治疗4周以及3个月、6个月后,对患者角化黏膜的宽度、龈缘外形、附着点的重建以及其他指标的改善状况运用SPSS 22.0统计学软件进行统计分析。结果 12组患者治疗后附着龈宽度对比发现:观察组患者的修复状况显著优于对照组,差异有统计学意义(P<0.05);22组患者治疗后龈缘外形与附着点的重建情况对比发现:观察组患者龈缘外形与附着点的重建协调率明显高于对照组,重建存在缺陷率明显低于对照组,差异有统计学意义(P<0.05)。结论附着龈重建术应用于口腔种植修复中可明显提高患者修复疗效,改善修复状况,提高重建协调率,降低缺陷发生情况。有利于患者口腔患部修复后牙龈的稳定;从美学方面考虑,附着龈重建术也具有十分重要的意义,且该手术方法具有简便易行的优点,具有十分重要的临床推广价值。
Objective To evaluate the clinical value of adhesive gingival reconstruction in oral implant repair by clinical control trial to provide reference for clinical practice. Methods A total of 120 patients admitted to our department from March 2010 to May 2014 were enrolled in this study. All 120 patients were divided into observation group and control group according to different treatment methods. Each group of 60 patients . Patients in the observation group underwent gingival reconstruction surgery, that is, the alveolar crest of the patient was incised at the side of the tongue with a positive bias toward the tongue. Patients in the control group were treated by traditional methods of incision in the middle part and gingival reconstruction Four weeks and three months and six months after treatment, the width of the keratomileusis, the shape of the gingival margin, the reconstruction of the attachment points and the improvement of other indexes were analyzed by SPSS 22.0 statistical software. Results The comparison of the gingiva width between the 12 groups after treatment showed that the repair status of the observation group was significantly better than that of the control group (P <0.05). The comparison of the appearance of gingival margin and the reconstruction of the attachment point in the 22 groups The results showed that the reconstructive coordination rate of gingival margin and attachment point in the observation group was significantly higher than that of the control group, and the defect rate of reconstruction was significantly lower than that of the control group (P <0.05). Conclusion The application of gingiva reconstruction in dental implants can significantly improve the efficacy of patients’ repair, improve the condition of repair, improve the coordination rate of reconstruction and reduce the occurrence of defects. Which is favorable for the stability of the gums after the oral cavity is repaired in the patient. It is also of great significance from the aesthetic point of view to attach the gingival reconstruction, and the operation method has the advantages of being simple and easy to be implemented and having very important clinical promotion value.
