目的探讨甘精胰岛素联合复方甲亢片治疗糖尿病并甲状腺功能亢进症(甲亢)的临床疗效。方法选取来宾市中西医结合医院2014年7月—2016年5月收治的糖尿病并甲亢患者96例,按照随机数字表法将患者分为对照组与观察组,各48例。对照组患者给予常规诺和灵R联合抗甲亢药物治疗,观察组患者给予甘精胰岛素联合复方甲亢片治疗,均持续治疗10 d。比较两组患者血糖达标时间、胰岛素用量、住院时间、血糖水平、甲状腺激素(促甲状腺激素、血清游离三碘甲腺原氨酸、甲状腺球蛋白抗体)水平及不良反应发生情况。结果观察组血糖达标时间、住院时间短于对照组,胰岛素用量少于对照组(P<0.05)。治疗前两组患者空腹血糖、餐后2 h血糖水平比较,差异无统计学意义(P>0.05);治疗后观察组空腹血糖、餐后2 h血糖水平低于对照组(P<0.05)。治疗前两组患者促甲状腺激素、血清游离三碘甲腺原氨酸、甲状腺球蛋白抗体水平比较,差异无统计学意义(P>0.05);治疗后观察组促甲状腺激素高于对照组,血清游离三碘甲腺原氨酸、甲状腺球蛋白抗体水平低于对照组(P<0.05)。观察组患者甲亢高代谢、低血糖发生率低于对照组(P<0.05)。结论甘精胰岛素联合复方甲亢片治疗糖尿病并甲亢的临床疗效确切,可有效患者控制血糖、甲状腺激素水平,其治疗时间短,不良反应少。
Objective To investigate the clinical efficacy of glargine combined with compound Ketang tablets in the treatment of diabetic patients with hyperthyroidism (hyperthyroidism). Methods Ninety-six patients with diabetes mellitus and hyperthyroidism admitted from July 2014 to May 2016 in Laibin Integrative Medicine Hospital were enrolled. According to the random number table, the patients were divided into control group and observation group, 48 cases each. Patients in the control group were given conventional Noropenem combined with anti-hyperthyroidism, and patients in the observation group were given insulin glargine combined with compound-Ketang tablets for 10 days. The duration of blood glucose, insulin dosage, hospital stay, blood glucose level, thyroid hormone (thyroid-stimulating hormone, serum free triiodothyronine, thyroglobulin antibody) levels and adverse reactions were compared between the two groups. Results The observation group had shorter blood glucose time, shorter hospital stay and less insulin than the control group (P <0.05). There was no significant difference in fasting blood glucose and blood glucose level between the two groups before treatment (P> 0.05). The fasting blood glucose and postprandial blood glucose 2 h after treatment in the observation group were lower than those in the control group (P <0.05). Thyroid stimulating hormone, serum free triiodothyronine and thyroglobulin antibody levels were not significantly different between the two groups before treatment (P> 0.05). The levels of thyrotropin in the observation group were higher than those in the control group Free triiodothyronine, thyroglobulin antibody levels were lower than the control group (P <0.05). Hyperthyroidism patients with high metabolic rate, hypoglycemia incidence was lower than the control group (P <0.05). Conclusion Glargine and Hyperthyroidism combined with Hyperthyroidism Tablet treatment of diabetes and hyperthyroidism clinical curative effect can be effective in patients with control of blood glucose and thyroid hormone levels, the treatment time is short, fewer adverse reactions.
