目的 探讨慢性肾衰 (CRF)并继发性甲旁亢 (SHPT)患者 ,观察肾功损害不同时期体内PTH、BUN、Cr、钙、磷、镁的变化规律。方法 选择CRF112例 ,正常对照 2 0例 ,以肾功能内生肌酐清除率 (Ccr)为指标 ,将CRF112例分为四组 :(A组 :Ccr2 0~5 0ml min 31例 ;B组 :Ccr≥ 2 0 <2 0ml min 4 0例 ;C组 :Ccr≥ 5 <10ml min 2 8例 ;D组 :Ccr<5ml min 13例 ) ,就血BUN、Cr、PTH、钙、磷、镁各组比较。结果 CRF组与正常对照组比较 ,PTH、BUN、Cr极具显著差异 (P <0 0 0 1) ;钙、磷、镁亦有显著性差异 (P <0 0 5 )。且PTH、BUN及Cr随肾功能损害程度的加重而增高 ,经相关分析 ,PTH与BUN呈显著正相关 (P <0 0 1) ;PTH与Cr极其显著正相关 (P <0 0 0 1)。结论 CRF并SHPT患者 ,观察血PTH的变化 ,直接反映肾功能损害程度及其预后 ,对指导相关治疗和提高CRF患者生存率有重要意义。
Objective To investigate the changes of PTH, BUN, Cr, Ca, P and Mg in patients with chronic renal failure (CRF) and secondary hyperparathyroidism (SHPT). Methods CRF112 cases were selected and 20 cases were normal controls. CRF112 cases were divided into four groups according to the renal function endogenous creatinine clearance (Ccr): (group A: Ccr2 0 ~ 50ml min 31 cases; group B: Ccr 8 cases with Ccr≥5 <10ml min 2, and 13 cases with Ccr <5ml min in group C), blood BUN, Cr, PTH, calcium, phosphorus and magnesium Compare Results There were significant differences in PTH, BUN and Cr between the CRF group and the normal control group (P <0.01), and there were significant differences between the two groups (P <0.05). PTH, BUN and Cr increased with the degree of renal dysfunction. Correlation analysis showed that there was a significant positive correlation between PTH and BUN (P <0.01); PTH and Cr were extremely significantly positively correlated (P <0.01) . Conclusion CRF and SHPT patients observed changes in blood PTH, a direct reflection of the extent of renal damage and prognosis, to guide the relevant treatment and improve the survival rate of patients with CRF is important.
