目的:该研究以肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)中医发病机制“肺络痹阻”为出发点,观察MPP患儿血中6-酮-前列腺素(6-Keto-PGFla)、血栓烷B_2(TXB_2)水平,探讨6-Keto-PGFla、TXB_2在MPP发病机制中的作用,阐释与“肺络痹阻”病机间相关性。方法:选取MPP患儿34例、病毒性肺炎患儿32例,细菌性肺炎患儿28例,检测3组6-Keto-PGFla、TXB_2水平、降钙素原(PCT)及血小板计数(PLT)。结果:MPP组患儿TXB_2水平与细菌组、病毒组患儿TXB_2水平比较差异均有统计学意义(均P<0.05),MPP组TXB_2含量高于细菌组、病毒组;病毒组PLT值与细菌组及MPP组比较差异有统计学意义,MPP组PLT值大于病毒组与细菌组。6-Keto-PGFla、6-Keto-PGFla/TXB_2、PCT各组比较差异均无统计学意义(P>0.05)。结论:MPP组缩血管因子降解产物TXB_2含量升高,提示肺炎支原体肺炎较细菌、病毒性肺炎凝血改变更为明显。患儿感染肺炎支原体后,肺内微循环障碍,破坏机体6-Keto-PGFla、TXB_2含量相对平衡。另外,外周血中血小板计数升高与支原体肺炎患儿血中TXB_2含量升高有密切关系。
Objective: To investigate the pathogenesis of “Myocardium pneumoniae pneumonia (MPP)” and “Liao Lu Bi Du” in children with MPP. The aim of this study was to investigate the effects of 6-Keto-PGFla, To investigate the role of 6-Keto-PGFla and TXB_2 in the pathogenesis of MPP, and to elucidate the relationship between them and the pathogenesis of “Pulmonary Tuberculosis”. Methods: Thirty-four children with MPP, 32 children with viral pneumonia and 28 children with bacterial pneumonia were enrolled in this study. The levels of 6-Keto-PGFla, TXB 2, PCT and PLT in 3 groups were detected. . Results: The level of TXB_2 in MPP group was significantly higher than that in bacterial group and virus group (all P <0.05). The content of TXB_2 in MPP group was higher than that in bacterial group and virus group. There was significant difference between MPP group and MPP group. PLT value of MPP group was higher than that of virus group and bacteria group. 6-Keto-PGFla, 6-Keto-PGFla / TXB_2, PCT in all groups showed no significant difference (P> 0.05). CONCLUSION: The increase of TXB_2, a product of vasoconstrictor in MPP, suggests that mycoplasma pneumonia is more obvious than bacteria and viral pneumonia. Children infected with Mycoplasma pneumoniae, pulmonary microcirculation, damage the body 6-Keto-PGFla, TXB_2 content is relatively balanced. In addition, elevated platelet count in peripheral blood and mycoplasma pneumonia in children with elevated blood levels of TXB_2 are closely related.
