目的:探究高压氧治疗在渔业腐气中毒中的临床疗效,并分析其对患者肺功能和血清炎性因子的影响。方法:选取2015年6月至2017年10月荣成市石岛人民医院收治的110例鳀鱼腐败气体中毒患者作为研究对象,依据治疗方法分为对照组(n n=55)和观察组(n n=55)。对照组给予常规对症支持治疗,观察组联合高压氧辅助治疗。观察2组患者临床治疗效果,比较治疗前后动脉血气分析、肺功能及炎性因子水平变化。n 结果:观察组患者临床总有效率为96.30%,显著高于对照组的81.82%,差异有统计学意义(n P<0.05);观察组格拉斯哥昏迷评分(GCS)、清醒时间及住院时间分别为(12.27±1.16)分、(3.41±1.06) h及(4.25±1.76) d,均明显优于对照组,差异有统计学意义(n P<0.05)。治疗后,2组患者动脉血氧饱和度(SaOn 2)、动脉氧分压(PaOn 2)和氧合指数(PaOn 2/FiOn 2)较治疗前显著上升,动脉二氧化碳分压(PaCOn 2)明显下降;且观察组PaOn 2及PaOn 2/FiOn 2改善优于对照组,差异有统计学意义(n P<0.05)。治疗后2组患者肺功能也有明显好转,观察组患者FEVn 1、FVC及FEVn 1/FVC显著高于对照组,差异有统计学意义(n P<0.05);血清炎性因子水平及白细胞计数较治疗前明显下降,且观察组IL-6、TNF-α、CRP水平较对照组显著降低,差异有统计学意义(n P<0.05)。n 结论:高压氧治疗能有效改善腐气中毒患者肺功能,减轻炎性反应,降低炎性因子水平,从而提高临床疗效。“,”Objective:To explore the clinical efficacy of hyperbaric oxygen in the treatment of fishery putrefaction gas poisoning, and to analyze its effects on lung function and inflammatory factors.Methods:A total of 110 patients with anchovy putrefaction gas poisoning admitted to Shidao People’s Hospital of Rongcheng from June 2015 to October 2017 were selected as study subjects and randomly divided into control group (n n=55) and observation group (n n=55). The control group was given conventional symptomatic and supportive treatment, while the observation group was treated with hyperbaric oxygen as an auxiliary treatment. The clinical curative effects of the two groups were observed. The changes in arterial blood gas, pulmonary function, and inflammatory factor levels were also compared before and after treatment.n Results:The total effective rate of the observation group was 96.30%, which was significantly higher than that (81.82%) of the control group (n P<0.05). The clinical Glasgow Coma Scale (GCS) score, awakening time, and the length of hospital stay of the observation group were (12.27±1.16), (3.41±1.06) h, and (4.25±1.76) d, respectively, all of which were significantly better than those of the control group (n P<0.05). After treatment, SaOn 2, PaOn 2, and PaOn 2/FiOn 2 of both groups increased significantly, while PaCOn 2 decreased significantly. PaOn 2 and PaOn 2/FiOn 2 in the observation group showed greater improvements than those in the control group (n P<0.05). After treatment, the lung function of patients in both groups improved, and the FEVn 1, FVC, and FEVn 1/FVC in the observation group were significantly higher than those in the control group (n P<0.05). The levels of serum inflammatory factors and white blood cell counts in both groups were significantly reduced than those before treatment, and the levels of IL-6, TNF-α, and CRP in the observation group were significantly lower than those in the control group (n P<0.05).n Conclusion:Hyperbaric oxygen therapy can effectively improve the lung function of patients with putrefaction gas poisoning, reduce inflammatory reactions and the levels of inflammatory factors, thus improving the total clinical efficacy.
