目的分析免疫球蛋白G(Ig G)在特发性膜性肾病(IMN)中的表达,并评估Ig G4与特发性膜性肾病24 h尿蛋白、血清白蛋白(ALB)等的关系。方法 97例经肾活检证实的IMN患者,应用免疫组化法检测肾组织Ig G,分析Ig G表达,并对Ig G4与IMN患者血清白蛋白、24 h尿蛋白量、血清总胆固醇等指标的关系进行分析。结果 97例IMN患者中Ig G4表达阳性率最高为95.9%,其后依次为Ig G1、Ig G2、Ig G3,阳性率分别为61.9%、24.7%、16.5%。Ig G4含量(+)组、(2+)组、(3+)组24 h尿蛋白含量分别为(1.53±0.87)、(2.75±1.12)、(6.84±2.01)g/L;血清白蛋白含量分别为(28.54±1.56)、(24.13±1.20)、(20.22±0.97)g/L。Ig G4(+)组与(2+)组、(2+)与(3+)组24 h尿蛋白比较,差异均有统计学意义(P<0.05);血清白蛋白差异也均有统计学意义(P<0.05)。结论 IMN患者病理组织Ig G4表达阳性率最高,其次为Ig G1;Ig G4表达强度与IMN患者24 h尿蛋白量、血清白蛋白存在一定关系,提示Ig G4可能在IMN的发病中起了重要作用,并与IMN患者的严重程度相关。
Objective To analyze the expression of immunoglobulin G (Ig G) in idiopathic membranous nephropathy (IMN) and to evaluate the relationship between Ig G4 and urinary albumin, serum albumin (ALB) and 24 h in patients with idiopathic membranous nephropathy. Methods Ninety-seven patients with IMN confirmed by renal biopsy were enrolled in this study. Immunohistochemistry was used to detect Ig G in renal tissue, Ig G expression was analyzed. Serum albumin, 24-hour urinary protein, serum total cholesterol and other indicators of patients with Ig G4 and IMN Relationship analysis. Results The highest positive rate of Ig G4 in 97 patients with IMN was 95.9%, followed by Ig G1, Ig G2 and Ig G3, the positive rates were 61.9%, 24.7% and 16.5% respectively. Urine protein contents of 24 h in Ig G4 (+), (2+) and (3+) groups were (1.53 ± 0.87), (2.75 ± 1.12) and (6.84 ± 2.01) g / (28.54 ± 1.56), (24.13 ± 1.20) and (20.22 ± 0.97) g / L, respectively. Conclusions The positive rate of Ig G4 expression in pathological tissues of IMN patients is the highest, followed by that of Ig G1. The expression intensity of Ig G4 is related to 24-hour urine protein and serum albumin in patients with IMN, suggesting that Ig G4 may play an important role in the pathogenesis of IMN , And correlated with the severity of IMN patients.
