目的:探讨青少年跟骨骨折患者经不同切口钢板内固定治疗的效果,以指导跟骨骨折患者手术方案的优化。方法:抽取郑州市骨科医院2019年1月至2020年5月收治的青少年跟骨骨折患者80例,根据随机数字表法将患者分为对照组和研究组,每组40例。对照组行传统L型切口钢板内固定术,研究组行跗骨窦切口联合跟腱旁纵行切口钢板内固定术。记录并比较两组患者的手术及恢复相关情况。术前及术后1、3个月,检测并比较两组跟骨解剖数据。术后3个月,比较两组足功能恢复情况;比较两组术后相关并发症发生情况。结果:术后3个月,两组足部功能恢复优良率相近(n P>0.05)。研究组术中出血量较少,手术时间、切口愈合时间、住院时间及骨折愈合时间均短于对照组(n P均<0.05)。术后1、3个月,研究组Bohler角、Gissane角、跟骨高度均高于对照组,跟骨宽度小于对照组(n P<0.05);研究组术后并发症发生率(5.00%,2/40)低于对照组(22.50%,9/40),n P0.05). The bleeding amount in the study group was less than that in the control group, and the operation time, incision healing time, hospital stay and fracture healing time were shorter than those in the control group (n P<0.05). The Bohler angle, Gissane angle, and calcaneus height in the study group were higher than those in the control group, 1 month and 3 months after surgery, and the calcaneus width in the study group was smaller than that in the control group (n P<0.05). The incidence of postoperative complications of the study group (5.00%, 2/40) was lower than that of the control group (22.50%, 9/40),n P<0.05.n Conclusions:For adolescent patients with calcaneal fractures, the surgical efficacy of the tarsal sinus incision combined with the longitudinal incision next to the Achilles tendon plate internal fixation is similar to that of the traditional L-shaped incision plate internal fixation. However, the operation time, incision healing time and fracture healing time of the former are shorter, which can bring better recovery of the physiological and anatomical structure of the calcaneus, and ideal overall benefit to patient.
