目的探讨IgA肾病与1-磷酸鞘氨醇及1-磷酸鞘氨醇受体1的相关性。方法选取2014年11月—2015年3月武汉大学人民医院经肾活检确诊为IgA肾病患者共15例为研究对象,并选取体检健康者10例为对照组。根据24h尿蛋白总量将IgA肾病患者分为少量蛋白尿组18例、大量蛋白尿组8例和中等程度蛋白尿组14例。酶联免疫吸附法测定血清和尿液1-磷酸鞘氨醇水平,免疫组化法检测肾脏组织1-磷酸鞘氨醇受体1表达。结果对照组血清1-磷酸鞘氨醇水平高于IgA肾病组(P<0.05),对照组和IgA肾病组尿液1-磷酸鞘氨醇水平差异无统计学意义(P>0.05)。不同程度尿蛋白组血清1-磷酸鞘氨醇水平差异有统计学意义(P<0.05),尿液1-磷酸鞘氨醇水平差异无统计学意义(P>0.05)。相关性分析显示,血清1-磷酸鞘氨醇水平与肌酐、24h尿蛋白总量、胆固醇和尿酸呈负相关(r值分别为-0.60、-0.57、-0.59和-0.72,P值分别为0.03、0.04、0.02和0.01),IgA肾病患者肾脏组织表达S1PR1较对照组低,且有统计学意义(P<0.05)。结论 IgA肾病患者血清1-磷酸鞘氨醇水平较低,且与肌酐、24h尿蛋白总量、胆固醇和尿酸呈负相关,IgA肾病患者肾脏组织1-磷酸鞘氨醇受体1表达减少。
Objective To investigate the correlation between IgA nephropathy and sphingosine-1-phosphate and sphingosine-1-phosphate-1. Methods A total of 15 patients with IgA nephropathy diagnosed by renal biopsy from November 2014 to March 2015 in Wuhan University People’s Hospital were enrolled as subjects and 10 healthy people were selected as the control group. IgA nephropathy patients were divided into a small amount of proteinuria group according to the total amount of 24-hour urinary protein in 18 cases, a large number of proteinuria group and 14 cases of moderate proteinuria group. Serum and urine sphingosine 1-phosphate levels were measured by enzyme-linked immunosorbent assay (ELISA), and the expression of sphingosine 1-phosphate 1 in renal tissue was detected by immunohistochemistry. Results The level of serum sphingosine-1-phosphate in the control group was higher than that in IgA nephropathy group (P <0.05). There was no significant difference in urine sphingosine 1-phosphate level between the control group and IgA nephropathy group (P> 0.05). There were significant differences in serum sphingosine-1-phosphate levels between different degrees of urinary albumin group (P <0.05) and urine sphingosine 1-phosphate levels (P> 0.05). Correlation analysis showed that serum sphingosine-1-phosphate levels were negatively correlated with creatinine, total 24-hour urinary protein, cholesterol and uric acid (r values were -0.60, -0.57, -0.59 and -0.72, P values were 0.03 , 0.04,0.02 and 0.01). The expression of S1PR1 in renal tissue of IgA nephropathy patients was lower than that of control group (P <0.05). Conclusions Serum sphingosine-1-phosphate is low in patients with IgA nephropathy and negatively correlated with creatinine, total urinary protein in 24 hours, cholesterol and uric acid. The expression of sphingosine 1-phosphate 1 -phosphate in renal tissue is decreased in IgA nephropathy patients.
