目的探讨原发性高血压患者踝臂指数(ABI)、肱踝脉搏波传导速度(ba PWV)和心脏结构、功能之间的相互联系,并分析其相关性。方法选取兰州军区兰州总医院2013年1月至2014年12月间收治的120例原发性高血压患者作为研究对象,测定患者ABI和baPWV,依据其不同值情况分为ABI降低(A组)和ABI正常(B组),baPWV增加(C组)和baPWV正常(D组),测定各组左心房前后径(LAD)、左心室舒张末期内径(LVDd)、左心室收缩末期内径(LVSd)、室间隔厚度(IVST)、左心室射血分数(LVEF)和A峰与E峰流速比值(E/A),并分析其相关性。结果 B组IVST低于A组,而LVEF、E/A均高于A组,差异有统计学意义(P<0.05)。D组LAD、IVST均明显低于C组,而LVEF和E/A均明显高于C组,差异有统计学意义(P<0.05)。ABI与LVEF、E/A呈正相关性(r=0.715、0.428,P<0.05),而与IVST呈负相关性(r=-0.318,P<0.05)。baPWV与LAD、IVST均呈正相关性(r=0.453、0.729,P<0.05),而与LVEF呈负相关性(r=-0.414,P<0.05)。结论原发性高血压患者ABI和baPWV均与心脏结构、功能改变有着密切联系,且呈相关性,可帮助医师对该病进行诊断与干预。
Objective To investigate the relationship between ABI, ba PWV and cardiac structure and function in patients with essential hypertension. METHODS: A total of 120 essential hypertension patients from Lanzhou General Hospital of Lanzhou Military Command from January 2013 to December 2014 were selected as the study object. ABI and baPWV were measured in patients with ABI and were divided into ABI (group A) (LAD), left ventricular end-diastolic dimension (LVDd), left ventricular end systolic diameter (LVSd) and left ventricular end diastolic dimension (LAD) were measured in all patients with normal ABI (group B), baPWV (group C) and normal baPWV (IVST), left ventricular ejection fraction (LVEF) and ratio of peak value of A to peak E (E / A), and analyzed its correlation. Results The IVST in group B was lower than that in group A, while LVEF and E / A were higher in group B than in group A (P <0.05). LAD and IVST in group D were significantly lower than group C, while LVEF and E / A in group D were significantly higher than those in group C (P <0.05). ABI was positively correlated with LVEF and E / A (r = 0.715,0.428, P <0.05), but negatively correlated with IVST (r = -0.318, P <0.05). There was a positive correlation between baPWV and LAD and IVST (r = 0.453, 0.729, P <0.05), but negatively correlated with LVEF (r = -0.414, P <0.05). Conclusion Both ABI and baPWV in patients with essential hypertension are closely related to changes in cardiac structure and function, and are correlated to help physicians diagnose and interfere with the disease.
