目的:双黄连粉针剂是治疗急性上呼吸道感染等感染性疾病的常用有效方剂,头孢拉定为头孢菌素类抗生素,两者在临床上的联合用药相当普遍。本课题旨在探讨双黄连粉针剂与头孢拉定联用后对肾功能的影响,为中药注射剂的评价研究及探索中西药相互作用研究、指导临床用药提供实验支持。方法:选用SD大鼠,分为空白对照组、双黄连组、头孢低剂量组、头孢中剂量组、头孢高剂量组、合并用药低剂量组、合并用药中剂量组、合并用药高剂量组。空白对照组尾静脉给生理盐水,双黄连组387mg/kg,头孢拉定高、中、低剂量组分别为830、415、207mg/kg,合并用药高剂量组为双黄连387㎎/㎏+头孢拉定830mg/kg;合并用药中剂量组为双黄连387mg/kg+头孢拉定415㎎/㎏;合并用药低剂量组为双黄连387mg/kg+头孢拉定207mg/kg。尾静脉给药一周后,取24h尿液,取血及肾脏,肾组织做病理检查。结果:尿蛋白、尿NAG、血肌酐和血尿素氮在合并用药三个组与空白对照组分别比较中统计学差异均显著;肾组织SOD、NAG、AKP的活力及MDA含量的改变有显著的剂量依赖关系。病理检查显示,合并用药组出现肾小球病变的情况强于单独给药组(头孢拉定),双黄连高剂量组出现病变情况较少。结论:合并用药较单独用药,对于肾脏组织在抗氧化作用、活性物质含量、肾组织细胞结构及功能上均有一定的影响,提示联合使用双黄连与头孢拉定对肾脏有一定毒性,且较单用组为强。
OBJECTIVE: Shuanghuanglian injection is a commonly used and effective prescription for the treatment of infectious diseases such as acute upper respiratory tract infection. The purpose of this project is to investigate the effect of Shuanghuanglian injection combined with cefradine on renal function, and to provide experimental support for the research on the evaluation of traditional Chinese medicine injection and the exploration of the interaction between Chinese and western medicine and the guidance of clinical drug use. Methods: SD rats were divided into blank control group, shuanghuanglian group, cefotaxime low dose group, cefotaxime dose group, cefotaxime high dose group, combined low dose group, combined middle dose group, combined with high dose group. Results: The urinary protein, urinary NAG, serum creatinine and blood urea nitrogen in the three groups were significantly lower than those in the blank control group. The activities of SOD, NAG, AKP and the contents of MDA in the kidney tissue were significantly different Dose-dependent relationship. Pathological examination showed that the incidence of glomerular lesions in the combination group was stronger than that in the group administered alone (cefradine), and the lesions in the Shuanghuanglian high-dose group were less likely. Conclusion: The combination therapy is better than single administration, which has a certain effect on the antioxidation, active substance content and the structure and function of kidney cells in the kidney, suggesting that the combined use of shuanghuanglian and cephradine has a certain toxicity to the kidney, Group is strong.
