目的探讨慢性阻塞性肺疾病(COPD)合并2型糖尿病急性加重期患者肺通气功能、肺弥散功能以及C反应蛋白与血糖的关联性。方法选取2011年~2014年我院住院的COPD急性加重期患者169例患者,分为单纯COPD组93例、CO PD合并2型糖尿病组76例,比较两组间肺通气功能、肺弥散功能以及C反应蛋白与血糖的关联性。结果 CO PD合并2型糖尿病组患者VC、FVC、FEV1、DLCO以及DLCO/VA均低于单纯COPD组(P<0.05);FEV1/FVC高于单纯COPD组(P<0.05)。糖化血清蛋白>2.8mmol/L组DLCO、DLCO/VA均低于糖化血清蛋白≤2.8mmol/L组(P<0.05)。C反应蛋白≥8.0mg/L组不同时期血糖均高于C反应蛋白<8.0mg/L组(P<0.05)。结论 CO PD合并2型糖尿病急性加重期时,患者肺功能受损,DLCO与糖化血清蛋白呈显著负相关,C反应蛋白与血糖之间呈正相关,提示患者血糖水平控制欠佳,肺功能受损严重。
Objective To investigate the relationship between pulmonary ventilation, pulmonary diffuse function, C-reactive protein and blood glucose in patients with chronic obstructive pulmonary disease (COPD) and acute exacerbation of type 2 diabetes mellitus (T2DM). Methods A total of 169 acute exacerbation COPD patients admitted to our hospital from 2011 to 2014 were divided into three groups: 93 cases in COPD group and 76 cases in CO PD group with type 2 diabetes mellitus group. The pulmonary function, lung diffusion function, Correlation between C-reactive protein and blood glucose. Results The VC, FVC, FEV1, DLCO and DLCO / VA in patients with type 2 diabetes mellitus complicated with CO PD were lower than those in patients with COPD alone (P <0.05). The FEV1 / FVC were higher than those in patients with COPD alone (P <0.05). The DLCO and DLCO / VA of glycated serum protein> 2.8mmol / L group were lower than those of glycosylated serum protein≤2.8mmol / L group (P <0.05). Blood glucose in C reactive protein ≥8.0mg / L group was higher than that in C reactive protein <8.0mg / L group (P <0.05). Conclusions CO 2 and type 2 diabetes mellitus are associated with impaired pulmonary function in COPD patients with acute exacerbation of type 2 diabetes. There is a significant negative correlation between DLCO and glycosylated serum protein and a positive correlation between C-reactive protein and blood glucose, suggesting poor control of blood glucose level and impaired pulmonary function serious.
