四面蓝光照射联合维生素E及布拉酵母菌治疗早产儿高胆红素血症的效果观察

目的:观察四面蓝光照射联合维生素E及布拉酵母菌治疗早产儿高胆红素血症的效果。方法:抽取2019年3月至2020年5月于商丘市中医院分娩的早产儿高胆红素血症患儿84例,依据随机数字表法分为对照组与观察组,每组42例。对照组采用四面蓝光照射联合维生素E治疗,观察组在对照组的基础上加用布拉酵母菌治疗,并随访1个月。观察比较两组黄疸出现时间、黄疸持续时间及黄疸消退时间。检测并比较两组治疗前及治疗1、3 d后的总胆红素及间接胆红素变化情况。记录比较两组发热、皮疹、腹胀、喂养不耐受等并发症发生情况。结果:观察组黄疸出现时间、黄疸持续时间及黄疸消退时间均短于对照组,差异有统计学意义(n P<0.05)。两组治疗1、3 d后的总胆红素、间接胆红素水平均较治疗前升高,但观察组治疗1、3 d后的总胆红素、间接胆红素水平均低于对照组,差异有统计学意义(n P0.05)。n 结论:四面蓝光照射联合维生素E及布拉酵母菌治疗早产儿高胆红素血症,可有效改善患儿的总胆红素及间接胆红素水平,缓解病情,且治疗期间不会增加不良反应发生率,安全可靠。“,”Objective:To observe the effects of four-sided blue light irradiation combined with vitamin E and Saccharomyces boulardii on hyperbilirubinemia in premature infants.Methods:A total of 84 premature infants with hyperbilirubinemia who were delivered in Shangqiu Hospital of Traditional Chinese Medicine from March 2019 to May 2020 were selected and divided into control group and observation group according to the random number table method, with 42 cases in each group. The control group was treated by four-sided blue light irradiation combined with vitamin E; while the observation group was treated with Saccharomyces boulardii based on the treatment of the control group. All patients were followed up for one month. The appearance time of jaundice, duration of jaundice and disappearing time of jaundice of the two groups were observed and compared. The changes of total bilirubin and indirect bilirubin of the two groups were detected and compared before treatment, 1 day after treatment, and 3 days after treatment. The incidence of complications, such as fever, rash, abdominal distension, and feeding intolerance, were recorded and compared between the two groups.Results:The appearance time of jaundice, duration of jaundice, and disappearing time of jaundice in the observation group were shorter than those in the control group, and the difference was statistically significant (n P<0.05). On 1 day and 3 days after treatment the levels of total bilirubin and indirect bilirubin of the two groups were higher than those before treatment, but the total bilirubin and indirect bilirubin levels of the observation group were lower than those of the control group (n P0.05).n Conclusions:Four-sided blue light irradiation combined with vitamin E and Saccharomyces boulardii in the treatment of hyperbilirubinemia in premature infants can effectively improve the levels of total bilirubin and indirect bilirubin, and relieve the disease condition, without increasing incidence of adverse reactions, which is safe and reliable.
