目的探讨 HER-2与 P53蛋白表达在乳腺癌中表达的相关性及临床意义。方法天津市肿瘤医院2002年7月至2005年11月住院治疗的2996例原发性乳腺癌,年龄15~92岁,中位50岁。均经手术切除病理组织学证实。采用免疫组化 SP 法进行雌激素受体(ER),孕激素受体(PR),表皮生长因子受体2(HER-2)及 P53蛋白检测。相关性计算采用卡方检验进行数据差异性分析,采用多元逐步回归进行多因素分析计算。结果 HER-2过表达阴性者2316例(77.3%),HER-2过表达阳性者680例(22.7%),其中 HER-2过表达弱阳性(++)378例(12.6%),强阳性(+++)302例(10.1%)。P53阳性者935例,P53阴性者2061例。HER-2过表达阳性在 P53阳性中占32.2%,在 P53阴性中占18.4%(P<0.01)。多因素统计学分析显示 HER-2表达与 P53表达呈正相关(P<0.01),即 p53表达阳性时,HER-2过表达阳性率增加。结论乳腺癌 P53的表达水平是HER-2表达的独立影响因素。
Objective To investigate the correlation and clinical significance of the expression of HER-2 and P53 protein in breast cancer. Methods Tumor Hospital of Tianjin from July 2002 to November 2005 hospitalized 2996 cases of primary breast cancer, aged 15 to 92 years, median 50 years. Histopathologically confirmed by surgical excision. Immunohistochemical SP method was used to detect estrogen receptor (ER), progesterone receptor (PR), epidermal growth factor receptor 2 (HER-2) and P53 protein. Correlation was calculated using chi-square test for data analysis of variance, using multivariate stepwise regression analysis of multivariate analysis. Results There were 2316 (77.3%) HER-2 overexpression negative and 680 (22.7%) HER-2 overexpression positive, 378 (12.6%) HER-2 overexpression weak positive (+++) 302 cases (10.1%). 935 cases of P53 positive, 2061 cases of P53 negative. The positive expression of HER-2 was 32.2% in P53 positive and 18.4% in P53 negative (P <0.01). Multivariate statistical analysis showed that there was a positive correlation between the expression of HER-2 and P53 (P <0.01), that is, the positive rate of HER-2 overexpression was increased when p53 was positive. Conclusion The expression of P53 in breast cancer is an independent factor of HER-2 expression.
