目的探讨动态增强MRI(DCEMRI)联合弥散加权成像(DWI)对子宫内膜癌诊断的准确性。方法选择2014年1月至2016年4月在本院治疗的子宫内膜癌确诊病例82例,采用DCE-MRI联合DWI对子宫内膜癌的肌层浸润情况及术前分期诊断,分析其灵敏度与特异度,并判断其术前分期与金标准的一致性。结果 DCE-MRI联合DWI获得正确诊断肌层浸润深度70例,准确率为95.89%;对浅肌层和深肌层浸润的灵敏度、特异度、阳性预测值和阴性预测值均高于90%。对75例病例的术前分期获得正确的诊断,准确率为91.46%,与金标准的一致性良好,Kappa指数为0.88(P<0.05)。结论 DCE-MRI联合DWI成像对子宫内膜癌的术前分期与肌层浸润深度的诊断具有较高的准确度,能改善病人的诊断和预后。
Objective To investigate the diagnostic accuracy of dynamic contrast-enhanced MRI (DCEMRI) and diffusion-weighted imaging (DWI) for endometrial cancer. Methods Totally 82 cases of endometrial cancer treated in our hospital from January 2014 to April 2016 were selected. DMI-MRI and DWI were used to diagnose the myometrial invasion and preoperative staging of endometrial carcinoma. The sensitivity And specificity, and determine the preoperative staging and gold standard consistency. Results The accuracy of DMI-MRI combined with DWI in correctly diagnosing myometrial invasion was 70% (95.89%). The sensitivity, specificity, positive predictive value and negative predictive value of superficial myometrial invasion were higher than 90%. The preoperative staging of 75 cases was correctly diagnosed, with an accuracy rate of 91.46%. The consistency with the gold standard was good. The Kappa index was 0.88 (P <0.05). Conclusion DCE-MRI combined with DWI imaging of endometrial cancer, preoperative staging and depth of myometrial invasion diagnosis with high accuracy, can improve the patient’s diagnosis and prognosis.
