目的:本研究旨在探讨COX-2、β-cat和MMP-7表达与HNPCC特殊侵袭转移生物学行为间的关系.方法:应用免疫组化SP法检测HNPCC(n=28)、散发性大肠癌(n=30)和正常大肠黏膜(n=10)中COX-2,β-cat和MMP-7的表达情况.所有标本预先经过hMSH2和hMLH1免疫组化染色筛查,并结合其临床病理特点进行回顾性分析.结果:在HNPCC组和散发性大肠癌组中COX-2、异位β-cat和MMP-7的表达差异显著(χ2=14.8352,P=0.0001;χ2=5.6425,P=0.0175;χ2=10.6454,P=0.0011).两组异位β-cat和MMP-7的阳性率与大肠癌的侵袭深度和淋巴结转移密切相关(P=0.0127,0.0001;P=0.0227,0.0261),而与性别、肿瘤的大小和部位无关.COX-2在HNPCC组中与肿瘤侵袭深度相关(P=0.0166),与淋巴结转移无关;散发性大肠癌组中与肿瘤侵袭深度和淋巴结转移均无关.两组中COX-2、异位β-cat和MMP-7三者阳性表达呈正相关(COX-2与异位β-cat:r=0.417,P=0.011,r=0.504,P=0.006;异位β-cat与MMP-7:r=0.396,P=0.027,r=0.429,P=0.021;COX-2与MMP-7:r=0.315,P=0.028,r=0.429,P=0.021).结论:COX-2、异位β-cat和MMP-7在HNPCC、散发性大肠癌和正常黏膜中的阳性表达率差异显著,这可能是HNPCC相对于散发性大肠癌侵袭弱、转移少的原因之一.
Objective: To investigate the relationship between the expression of COX-2, β-cat and MMP-7 and biological behavior of HNPCC in special invasion and metastasis.Methods: Immunohistochemical SP method was used to detect the expression of HNPCC (n = 28) The expression of COX-2, β-cat and MMP-7 in carcinoma (n = 30) and normal colorectal mucosa (n = 10) were detected by immunohistochemical staining of hMSH2 and hMLH1 in combination with their clinicopathological features (Χ2 = 14.8352, P = 0.0001; χ2 = 5.6425, P = 0.000, respectively) in HNPCC group and sporadic colorectal cancer group.Results: The expression of COX-2, ectopic β-cat and MMP- 0.0175; χ2 = 10.6454, P = 0.0011). The positive rates of ectopic β-cat and MMP-7 in both groups were closely related to the invasiveness and lymph node metastasis of colorectal cancer (P = 0.0127,0.0001; P = 0.0227,0.0261) COX-2 was correlated with tumor invasion depth in HNPCC group (P = 0.0166), but not with lymph node metastasis. There was no correlation between COX-2 and tumor invasion depth and lymph node metastasis in sporadic colorectal cancer group. There was a positive correlation between COX-2 and ectopic β-cat: r = 0.417, P = 0.011, r = 0.504, P = 0.006 Β-cat and MMP-7: r = 0.39 6, P = 0.027, r = 0.429, P = 0.021; COX-2 and MMP-7: r = 0.315, P = 0.028, r = 0.429, And MMP-7 in HNPCC, sporadic colorectal cancer and normal mucosa, which may be one of the reasons that HNPCC has less invasion and less metastasis than sporadic colorectal cancer.
