目的探讨冠心病合并糖调节受损患者的冠状动脉病变特点及临床特征。方法回顾分析我院2010-2011年冠状动脉造影阳性患者125例,分为糖代谢正常组(35例)、糖调节受损组(51例)、糖尿病组(39例)、所有患者均测定空腹血糖、餐后2h血糖、总胆固醇、甘油三脂、低密度脂蛋白、高密度脂蛋白、尿素氮、肌酐,观察其临床特征:年龄、性别、冠心病危险因素(包括高血压、高胆固醇血症、体重指数和吸烟)和冠状动脉病变情况有无差异。结果 3组患者在一般情况(年龄、性别、吸烟、高血压)等方面差异无统计学意义(P>0.05),而BMI、FBG、TG水平升高(P<0.05),HDL-C水平降低(P<0.05),有显著性差异;糖调节受损组及糖尿病组与糖代谢正常组比较,糖调节受损组及糖尿病组冠脉病变多为多支病变,有显著性差异(P<0.05),而糖调节受损组和糖尿病组间多支血管病变的患病率的差异无统计学意义(P均>0.05)。结论糖调节受损时冠状动脉已发生严重病变,病变呈弥漫性,与糖尿病组相近,应用口服葡萄糖耐量试验及时发现糖代谢异常患者非常重要。
Objective To investigate the characteristics and clinical features of coronary lesions in patients with impaired glucose regulation and coronary heart disease. Methods A retrospective analysis of 125 cases of positive coronary angiography in our hospital from 2010 to 2011 were divided into normal glucose metabolism group (35 cases), impaired glucose regulation group (51 cases) and diabetic group (39 cases). All patients were tested for fasting Blood glucose, 2h postprandial blood glucose, total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein, urea nitrogen and creatinine were observed. The clinical features were observed: age, gender, coronary heart disease risk factors (including hypertension, hypercholesterolemia Disease, body mass index and smoking) and coronary artery disease whether there is any difference. Results There were no significant differences in general conditions (age, gender, smoking, hypertension) between the three groups (P> 0.05), while the levels of BMI, FBG and TG increased (P <0.05). There was a significant difference between the two groups (P <0.05). There was significant difference between diabetic impaired glucose tolerance group and diabetes mellitus group (P < 0.05). However, there was no significant difference in the prevalence of multivessel disease between the impaired glucose regulation group and diabetic group (all P> 0.05). Conclusions Severe coronary artery disease has occurred in impaired glucose regulation, and the pathological changes are diffuse. It is very similar to diabetes group. It is very important to detect oral glucose tolerance test in time by oral glucose tolerance test.
