目的:评价高强度聚焦超声治疗晚期胰腺癌患者的有效性,并进行预后相关因素分析。方法:对80例无法手术的晚期胰腺癌患者实施高强度聚焦超声治疗,评价近期疗效;应用Kaplan-Meier法进行生存分析,计算无进展生存时间和无进展生存率;log-rank检验进行预后的单因素分析,COX回归模型进行预后的多因素分析。结果:80例患者的有效率为5.0%,疾病控制率为70.0%。中位无进展生存时间为6个月(范围:0~22个月);6个月、1年和2年无进展生存率分别为45.0%、12.5%和0.0%。TNM分期、有无肝转移和肝功能是无进展生存的独立预后因素(P值均<0.05)。结论:高强度聚焦超声治疗能在一定程度上延长晚期胰腺癌患者的无进展生存时间;而TNM分期、有无肝转移和肝功能是无进展生存的独立预后因素。
Objective: To evaluate the effectiveness of high intensity focused ultrasound in the treatment of patients with advanced pancreatic cancer and to analyze prognostic factors. Methods: Eighty patients with unresectable advanced pancreatic cancer underwent high-intensity focused ultrasound to evaluate the short-term curative effect. Survival analysis was performed by Kaplan-Meier method to calculate the progression-free survival time and progression-free survival rate. Log-rank test was used to evaluate prognosis Univariate analysis, COX regression model for prognosis of multivariate analysis. Results: In 80 patients, the effective rate was 5.0% and the disease control rate was 70.0%. The median progression-free survival time was 6 months (range, 0-22 months); the 6-month, 1-year and 2-year progression-free survival rates were 45.0%, 12.5% and 0.0%, respectively. TNM staging, presence or absence of liver metastases, and liver function were independent prognostic factors for progression-free survival (P values <0.05). CONCLUSION: High-intensity focused ultrasound can prolong the progression-free survival time of patients with advanced pancreatic cancer to a certain extent. TNM stage, liver metastasis and liver function are independent prognostic factors of progression-free survival.
