目的探讨应用异种(牛)脱细胞真皮基质修复膜修复喉部手术后黏膜缺损的临床使用价值。方法对20例喉癌术后的黏膜缺损应用异种脱细胞真皮基质修复膜1期修复,其中声门区癌18例,其中T2N0M0 8例,T3N1MO 5例,T3N2M0 4例,T4N2MO 1例。声门上区癌2例,T3N1M0 1例,T3N2MO 1例。结果20例患者黏膜缺损修复均1期愈合,患者术后3~6个月应用电子喉镜复查,修复膜均已替代缺损黏膜组织,局部功能恢复;20例喉癌中6月生存率100%(11/11);一年生存率100%(3/3),其余9例术后不足6月,正在观察之中。讨论异种脱细胞基质修复膜为修复喉肿瘤术后的黏膜缺损提供了一种新的方法,手术操作简便,有很好的临床应用价值。
Objective To investigate the clinical value of using xenogenic (bovine) acellular dermal matrix to repair mucosal defects after laryngeal surgery. Methods Twenty cases of mucosal defects of laryngeal carcinoma treated with xenografted acellular dermal matrix were repaired in one stage. Among them, there were 18 cases of glioma, including 8 cases of T2N0M0, 5 cases of T3N1MO, 4 cases of T3N2M0 and 1 case of T4N2MO. 2 cases of supraglottic carcinoma, 1 case of T3N1M0 and 1 case of T3N2MO. Results All the mucosal defects were repaired in the first phase in 20 cases. The patients were examined by electronic laryngoscope 3 to 6 months after operation. The repaired membrane replaced the defect mucosa tissue and the local function recovered. The survival rate of 20 cases of laryngeal carcinoma in June was 100% (11/11); one-year survival rate was 100% (3/3), and the remaining 9 cases were less than 6 months after surgery and were under observation. Discussion The dissimilar acellular matrix repair membrane provides a new method for the repair of mucosal defects after laryngeal neoplasms surgery. The operation is simple and has good clinical value.
