目的探讨应用液质联用(high performance liquid chromatography-mass spectrometry,HPLC-MS)技术分析尿酸氧化酶体外溶解痛风石的疗效。方法收集行痛风石切除治疗患者的痛风石标本,将标本纯化、风干、称质量处理后分为实验组和对照组,实验组采用尿酸氧化酶1 mg/mL溶解处理,对照组采用质量分数5%碳酸氢钠溶解处理,应用HPLC-MS技术检测2组溶解产物尿囊素含量,称量2组溶解后痛风石质量。结果实验组检出明显尿囊素色谱峰,定量尿囊素浓度为(0.377±0.090)μmol/L,对照组尿囊素峰不明显,未定量检测出浓度;实验组溶解后痛风石质量[(437.7±1.7)mg]明显低于溶解前[(549.7±0.8)mg](P<0.01),对照组溶解后痛风石质量[(523.2±4.7)mg]低于溶解前[(549.2±0.8)mg](P<0.05);实验组溶解前后痛风石质量减少绝对值[(112.0±1.9)mg]明显高于对照组[(26.0±4.1)mg](P<0.01)。结论尿酸氧化酶与质量分数5%碳酸氢钠溶液在体外条件下均对痛风石的溶解有一定作用,但尿酸氧化酶溶解效果优于5%碳酸氢钠溶液。
Objective To investigate the efficacy of urate oxidase in the dissolution of tophi in vitro by high performance liquid chromatography-mass spectrometry (HPLC-MS). Methods The specimens of toxins from patients who had been treated with toadstones were collected. The specimens were purified and air-dried. The specimens were divided into experimental group and control group. The experimental group was treated with uric acid oxidase 1 mg / mL, while the control group was treated with mass fraction 5 % Sodium bicarbonate solution, HPLC-MS technique was used to detect the content of allantoin in two groups of dissolution products, and the quality of tophi was measured in two groups after dissolution. Results Significant allantoin peak was detected in the experimental group, the concentration of allantoin was (0.377 ± 0.090) μmol / L, while the allantoin peak in the control group was not obvious. The concentration of allantoin was not detected in the experimental group [ (437.7 ± 1.7) mg] was significantly lower than that before dissolution [(549.7 ± 0.8) mg] (P <0.01), and the quality of tophi was significantly lower than that before dissolution [(523.2 ± 4.7) mg] ) mg] (P <0.05). The absolute value of the toe stone mass decrease in the experimental group was significantly higher than that in the control group [(112.0 ± 1.9) mg] [(26.0 ± 4.1) mg] (P <0.01). Conclusion Uricase oxidase and 5% sodium bicarbonate solution have some effects on the dissolution of tophi in vitro, but uricase oxidase is better than 5% sodium bicarbonate solution.
