目的探究弥漫大B细胞淋巴瘤(DLBCL)患者的c-Myc,Bcl-2,Ki-67在组织中的表达及其临床意义。方法选取潍坊市人民医院2013年2月~2016年2月确诊的80例弥漫性大B细胞淋巴瘤患者的淋巴瘤组织纳入试验组,另选取本院同期80例健康志愿者的淋巴组织纳入对照组。采用免疫组织化学法检测DLBCL患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白表达情况,统计分析其与DLBCL患者临床特征之间的关系。结果试验组患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白的阳性表达率分别为63.75%,72.50%,58.75%,对照组分别为37.50%,41.25%,28.75%,两组比较差异显著(P<0.05);c-Myc,Bcl-2,Ki-67在不同性别、年龄及不同临床分期的DLBCL患者中阳性表达率无显著差异(P>0.05)。国际标准化预后指数(IPI)评分3~5分、结外受侵、肿瘤未完全缓解的DLBCL中,c-Myc,Bcl-2蛋白的阳性表达显著高于IPI评分0~2分、非结外受侵、肿瘤完全缓解的患者(P<0.05)。Ki-67的表达在IPI评分、Hans免疫分型、临床疗效、乳酸脱氢酶(LDH)水平中存在显著差异(P<0.05)。结论 DLBCL患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白的表达水平较高,且常见于肿瘤未完全缓解及IPI评分较高的患者。
Objective To investigate the expression of c-Myc, Bcl-2 and Ki-67 in patients with diffuse large B-cell lymphoma (DLBCL) and its clinical significance. Methods Lymphoma tissues of 80 patients with diffuse large B-cell lymphoma confirmed by Weifang People’s Hospital from February 2013 to February 2016 were enrolled in the experimental group. Lymphoid tissues from 80 healthy volunteers in the same period of our hospital were included in the control group group. The expression of c-Myc, Bcl-2 and Ki-67 in lymphoid tissue from patients with DLBCL was detected by immunohistochemistry. The relationship between them and the clinical features of DLBCL was statistically analyzed. Results The positive expression rates of c-Myc, Bcl-2 and Ki-67 in the experimental group were 63.75%, 72.50% and 58.75%, respectively, while those in the control group were 37.50%, 41.25% and 28.75% respectively (P <0.05). The positive rates of c-Myc, Bcl-2 and Ki-67 in DLBCL patients of different gender, age and different clinical stages were not significantly different (P> 0.05). The international normalized prognosis index (IPI) score 3 to 5 points, the extranodal invasion, the tumor did not completely relieve the DLBCL, c-Myc, Bcl-2 protein was significantly higher than the IPI score 0 to 2 points, Invaded, patients with complete tumor remission (P <0.05). The expression of Ki-67 in IPI score, Hans immunophenotype, clinical efficacy, lactate dehydrogenase (LDH) levels were significantly different (P <0.05). Conclusion The expression of c-Myc, Bcl-2 and Ki-67 in lymphoid tissue of patients with DLBCL is high, and it is common in patients with incomplete tumor remission and high IPI score.
