70%未经治疗的结节病患者血清血管紧张素转化酶(ACE)增高。ACE在结节肉芽肿的类上皮细胞内形成。类皮质激素药物可阻止肺泡炎和肉芽肿的新形成。用类皮质激素治疗结节病患者,几乎在所有病例均导致患者增高的ACE水平相应迅速下降,且常伴临床症状改善。故治疗结节病时ACE是项很有益的指标。大多数研究认为,用药4周后ACE始能被控制。为判断首次治疗时强地松的最佳剂量,作者用不同剂量强地松治疗16例Ⅱ期结节病患者,以观察患者ACE下降起始过程是否与药物剂量有关。所有病例均
Seventy percent of untreated sarcoidosis patients had elevated serum angiotensin-converting enzyme (ACE). ACE is formed in the nodular granulomatous epithelial cells. Corticosteroids prevent new formation of alveolitis and granulomas. Treatment of patients with sarcoidosis with corticosteroids resulted in a correspondingly rapid decrease in patients with elevated ACE levels in almost all cases, often accompanied by an improvement in clinical symptoms. Therefore, the treatment of sarcoidosis ACE is a very useful indicator. Most studies suggest that ACE can be controlled after 4 weeks of treatment. In order to determine the optimal dose of prednisone for the first time, the authors treated 16 patients with stage Ⅱ sarcoidosis with different doses of prednisone in order to observe whether the onset of ACE decline was related to the dosage of the drug. All cases are
