目的评价一种新的羊膜缝合方法——连续交锁缝合,在眼表羊膜移植术中应用及其临床疗效。方法回顾总结我院行羊膜移植术的眼表疾病患者57例(65眼),观察组26例(31眼)行连续交锁缝合,与对照组31例(34眼)行间断或单纯连续缝合者进行比较,随访时间均超过2m,观察两组羊膜脱落时间、创面上皮化时间等指标。结果两组3w内羊膜自行脱落眼数比:观察组3/31眼明显低于对照组为15/34眼,两者之间的差别有统计学意义(P=0.018);羊膜完整存留3w以上眼数比:观察组19/22眼高于对照组6/21眼,两者之间的差别有统计学意义(P=0.044);两组2w内角膜上皮化眼数比:观察组17/28眼高于对照组12/32眼,但两者之间的差别无统计学意义(P=0.29)。结论连续交锁缝合可使羊膜紧贴眼表,防止其融解脱落,并促进眼表创面上皮化,其眼表刺激症状很小,可明显提高羊膜眼表覆盖的疗效。
Objective To evaluate a new amniotic suture method - continuous interlocking suture in the amniotic membrane transplantation and its clinical efficacy. Methods A total of 57 patients (65 eyes) with ocular surface diseases undergoing amniotic membrane transplantation in our hospital were retrospectively reviewed. Twenty-six eyes (31 eyes) in the observation group underwent continuous interlocking suture and 31 cases (34 eyes) Who were followed up for more than 2m, observed amniotic membrane shedding time, wound epithelialization time and other indicators. Results The rate of spontaneous exfoliation of amniotic membrane in 3 weeks in both groups was significantly lower than that in control group (3/31 eyes: 15/34 eyes, P = 0.018), and the amniotic membrane remained intact for 3 weeks The number of eyes in the observation group was higher than that in the control group (19/22), while the difference was statistically significant (P = 0.044) 28 eyes were higher than the control group 12/32 eyes, but the difference between the two was not statistically significant (P = 0.29). Conclusions Continuous interlocking suture can make the amniotic membrane close to the ocular surface to prevent it from melting and exfoliation and promote the epithelization of the ocular surface. The ocular surface irritation is very small, which can obviously improve the efficacy of amniotic surface covering.
