循证医学中系统评价的结果作为卫生决策的依据,影响到医疗实践、卫生决策、医疗科研和新药开发等方面。系统评价本身面临着可靠性问题,其中最难克服的偏倚就是出版偏倚。出版偏倚对循证医学的结论会产生致命的影响,然而出版偏倚却是一个普遍存在的现象。产生出版偏倚的原因甚多,有研究报道者不正确的观念、编辑出版中选择过程和出版商的观念因素等。为从根本上解决出版偏倚问题,需要研究者、报道者和编辑出版者共同参与,从各个环节上加以控制,以从源头上减少出版偏倚的发生。
The results of systematic reviews in evidence-based medicine serve as the basis for health decision-making, affecting aspects of medical practice, health policymaking, medical research and new drug development. The systematic review itself is subject to reliability issues, of which the most insurmountable bias is publication bias. Publication bias has a fatal impact on EBM conclusions, but publication bias is a ubiquitous phenomenon. There are many reasons for the publication bias, the incorrectness of the reporter, the selection process of the editor and the concept of the publisher. In order to solve the problem of publication bias fundamentally, researchers, reporters and editors and publishers need to be involved and controlled from all aspects to reduce the source of publication bias.
