目的探讨α L 岩藻糖苷酶 (AFU) ,甲胎蛋白 (AFP)及其异质体 (AFP L3)在原发性肝癌诊断中的意义。方法设定原发性肝癌(PHC)组 ,慢性肝炎肝硬变组 ,正常对照组 ,测定各组的AFU、AFP及AFP L3。AFU采用比色法 ,AFP采用电化学发光法 ,AFP L3采用扁豆凝集素 (LCA)亲合电泳印迹法。结果在 1 0 0例PHC患者中 ,AFP >2 0ng/ml的阳性率为 68.0 % ,AFU阳性率为 76 .0 % ,AFP L3阳性率为 78.0 %。AFP <2 0ng/ml的 2 0例PHC患者中 ,AFU阳性率为 70 .0 % ,AFP L3阳性率为 1 0 .0 % ;AFP在 2 0~ 40 0ng/ml的 30例PHC患者中 ,AFU阳性率为 80 .0 % ,AFP L3阳性率为 90 .0 % ;AFP >40 0ng/ml的 50例PHC患者中 ,AFU阳性率为 76 .0 % ,AFP L3阳性率为98.0 % ,三者联合应用的PHC患者中阳性者 91例 ,阳性率为 91 .0 % ,三者同时阳性者 55例 ,阳性率为 55 .0 %。结论AFU、AFP L3联合检测在AFP <40 0ng/ml的PHC患者中可优势互补 ,阳性率显著提高 ,尤其是对小肝癌的早期诊断及其与良性肝病的鉴别诊断具有重要价值
Objective To investigate the significance of α-L-fucosidase (AFU), AFP and AFP L3 in the diagnosis of primary liver cancer. Results In 100 PHC patients, the positive rate of AFP> 20ng / ml was 68.0%, the positive rate of AFU was 76.0%, and the positive rate of AFP L3 was 78.0%. In 20 PHC patients with AFP <20 ng / ml, the positive rate of AFU was 70.0% and the positive rate of AFP L3 was 1.0%.
