作者采用口服环丙沙星成功治愈21例恶性外耳炎(Malignant external otitis,MEO)。方法本组23例MEO患者,其中16例伴有糖尿病。男性16例、女性7例,年龄54～92岁(平均72.7岁)。本组患者均符合MEO标准,即严重外耳道感染对常规保守治疗无效至少10天;耳道内伴有严重局部炎症及典型肉芽组织;夜间疼痛加重;多次细菌培养证实革兰氏阴性致病菌。口服环丙沙星每天1.5～2.25 g,用药6周;用药72小时后再行局部清创术。
The authors successfully treated 21 cases of malignant otitis externa (MEO) with oral ciprofloxacin. Methods 23 patients with MEO in this group, of which 16 were associated with diabetes mellitus. 16 males and 7 females, aged 54 to 92 years (average 72.7 years). This group of patients are in line with the MEO standard, that serious external auditory canal infection conservative treatment of conventional ineffective at least 10 days; accompanied by severe local inflammation of the ear canal and typical granulation tissue; aggravated night pain; multiple bacterial cultures confirmed Gram-negative pathogens. Oral ciprofloxacin 1.5 ~ 2.25 g daily for 6 weeks; local debridement 72 hours after treatment.
