目的探讨DWI对脑转移瘤的组织学特征诊断价值。方法分析33例不同病理类型的脑转移瘤DWI信号的差别,并在ADC图上计算其相对ADC值(rADC),分析不同组织学类型脑转移瘤rADC之间的区别。结果在DWI上高分化腺癌脑转移瘤倾向于显示低信号,小细胞和大细胞神经内分泌癌脑转移瘤呈现高信号。高分化腺癌脑转移瘤DWI上平均信号强度明显低于低分化腺癌脑转移瘤及其他病理类型脑转移瘤(P<0.05),高分化腺癌脑转移瘤平均rADC值明显高于低分化腺癌及其他病理类型的脑转移瘤(P<0.05)。结论不同病理类型的脑转移瘤在DWI和ADC图上具有相应的特征,DWI有助于对其组织学类型进行评估。
Objective To investigate the diagnostic value of DWI on the histological features of brain metastases. Methods The differences of DWI signals in 33 cases with different pathological types of brain metastases were analyzed. The relative ADC values (rADC) were calculated on ADC maps. The differences between rADCs in different histological types of brain metastases were analyzed. Results Highly differentiated adenocarcinomas of brain metastases on DWI tended to show low signals, and small cells and large cell neuroendocrine carcinomas showed high signal on brain metastases. The average signal intensity of DWI in well-differentiated adenocarcinoma was significantly lower than that in poorly differentiated adenocarcinoma and other pathological types of brain metastases (P <0.05). The average rADC of brain metastases in well-differentiated adenocarcinoma was significantly higher than that in poorly differentiated adenocarcinoma Adenocarcinoma and other pathological types of brain metastases (P <0.05). Conclusions Different pathological types of brain metastases have corresponding features on DWI and ADC maps. DWI is helpful to evaluate their histological types.
