目的探讨癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、唾液酸(SA)、腺苷脱氨酶(ADA)对良恶性胸水的鉴别诊断。方法检测经确诊的45例良性胸水和47例恶性胸水的CEA、CA19-9、SA、ADA的含量。结果单纯检测CEA、CA19-9、SA诊断恶性胸水的敏感性分别是:74.5%、72.3%、72.3%;特异性为88.9%、91.2%、86.4%;准确度为81.5%、81.5%、79.3%。ADA诊断良性胸水的敏感性73.3%,特异性91.5%,准确度为80.2%。四项联合检测其敏感性57.7%,特异性为73.4%,准确度为80.2%。结论CEA、CA19-9、SA、ADA联合检测对良恶胸水有较高鉴别诊断价值。
Objective To investigate the differential diagnosis of benign and malignant hydrothorax with CEA, CA19-9, SA and ADA. Methods The levels of CEA, CA19-9, SA, ADA in 45 cases of benign pleural effusion and 47 cases of malignant pleural effusion were detected. Results The sensitivity of detecting CEA, CA19-9 and SA in the diagnosis of malignant pleural effusion were 74.5%, 72.3% and 72.3% respectively; the specificity was 88.9%, 91.2% and 86.4% respectively; the accuracy was 81.5%, 81.5% and 79.3 %. ADA diagnosis of benign pleural effusion sensitivity 73.3%, specificity 91.5%, accuracy of 80.2%. The four combined tests showed a sensitivity of 57.7%, a specificity of 73.4% and an accuracy of 80.2%. Conclusion CEA, CA19-9, SA, ADA combined detection of benign and malignant pleural effusion have a higher differential diagnosis value.
