目的:探讨弹性应变比值联合超声乳腺影像报告和数据系统(BI-RADS)在乳腺小肿块定性诊断中的应用价值。方法:以常规超声诊断为BI-RADS 2~4级实性乳腺肿块120个为研究对象,在弹性成像模式下对所有肿块进行弹性应变比值检测,综合2种方法作出定性诊断,并与病理诊断对照分析。结果:120个乳腺肿块病理诊断结果均为良性,其中纤维腺瘤79个,乳腺腺病39个,导管内乳头状瘤1个,脂肪坏死1个。BI-RADS分级情况:2级4个,3级76个,4级40个。120个乳腺肿块弹性应变比值为0.43~3.04,平均应变比值1.78±1.30。联合应用BI-RADS评分和弹性应变比值法诊断120个乳腺肿块均为良性,诊断符合率100%。结论:超声弹性成像联合BI-RADS能够提高对乳腺小肿块的定性诊断价值,可为微创旋切选择合适的病例。
Objective: To investigate the value of elastic strain ratio combined with ultrasound breast imaging report and data system (BI-RADS) in the qualitative diagnosis of small breast lumps. Methods: Totally 120 BI-RADS solid tumors of grade 2 to 4 were diagnosed by conventional ultrasound. The elasto-elastic strain ratio of all masses was measured under elastic imaging mode. Two methods were used to make qualitative diagnosis and were compared with pathological diagnosis Control analysis. Results: The pathological diagnosis of 120 breast masses was benign, including 79 fibroadenoma, 39 breast adenosis, 1 intraductal papilloma and 1 fat necrosis. BI-RADS Classification: Level 2 4, Level 3 76, Level 4 40. The elastic strain ratio of 120 breast masses was 0.43 ~ 3.04, and the average strain ratio was 1.78 ± 1.30. The combined application of BI-RADS score and elastic strain ratio method in the diagnosis of 120 breast masses were benign, the diagnostic coincidence rate of 100%. Conclusion: Elastography combined with BI-RADS can improve the diagnostic value of small breast masses, and can select the appropriate cases for minimally invasive rotation.
