目的探讨正常高值血压者冷加压试验血压变化与24h血压变异相关性。方法将受试者261人按血压水平分为理想血压组、正常高值血压组和高血压组。所有受试者均进行冷加压试验、24h动态血压监测及血液生化指标检测。结果正常高值血压组三酰甘油、空腹血糖高于理想血压组(P<0.05);正常高值血压者冷加压后0及60s收缩压增加幅度分别为(14.0±6.5)及(9.3±5.0)mmHg,冷加压试验阳性率为25.6%,低于高血压组,但明显高于理想血压组(P<0.05);正常高值血压组24h、白天及夜间收缩压、舒张压变异均明显高于理想血压组(P<0.05);正常高值血压组0s收缩压增加幅度与夜间收缩压变异、24h收缩压变异呈正相关(r=0.625和r=0.512,均P<0.01);60s收缩压增加幅度与夜间收缩压、舒张压变异呈正相关(r=0.617和r=0.533,均P<0.01)。结论正常高值血压者交感神经系统活性增强,冷加压后血压增加幅度上升,血压变异性增加,冷加压后血压增幅与血压变异相关。
Objective To investigate the correlation between the change of blood pressure and the variation of 24 h blood pressure in cold stress test of normal high blood pressure. Methods The 261 subjects were divided into ideal blood pressure group, normal high blood pressure group and high blood pressure group according to the blood pressure level. All subjects were cold pressure test, 24h ambulatory blood pressure monitoring and biochemical detection of blood. Results Compared with ideal blood pressure group, triglyceride and fasting blood glucose in normal high blood pressure group were significantly higher than those in ideal blood pressure group (P <0.05). The increase of systolic blood pressure at 0 and 60 s in patients with normal high blood pressure were (14.0 ± 6.5) and (9.3 ± 5.0) mmHg, the positive rate of cold compression test was 25.6%, lower than that of hypertension group, but significantly higher than that of ideal blood pressure group (P <0.05); The changes of systolic and diastolic blood pressure (P <0.05). The increase rate of systolic blood pressure at 0s in normal high blood pressure group was positively correlated with the variation of systolic blood pressure at night and systolic blood pressure at 24h (r = 0.625 and r = 0.512, all P <0.01) Systolic blood pressure increased amplitude and night systolic blood pressure, diastolic blood pressure was positively correlated (r = 0.617 and r = 0.533, both P <0.01). Conclusions The normal high blood pressure increased the activity of the sympathetic nervous system, the increase of blood pressure after cold pressurization increased, and the variation of blood pressure increased. The increase of blood pressure after cold pressurization was related to the variation of blood pressure.
